Well your link works, and 16 pressure did not result in an improvement, and no worse either. The chart sure looks like you are having airway obstruction in clusters through the night. I think trying the soft cervical collar may be worthwhile. I have seen them at the drug stores as well as Amazon. I think you could throw a full 20 cm of pressure at this and still not fully overcome the obstruction. If you don't see some positive change pretty soon, going back to fixed pressure at your prescription, and consulting your doctor may be the next step.

Well I see a slight improvement, it looks like less time against the limit. On you new graph look at a line where your old limit was and you will see a shorter time below that limit than now. It does look like you slept through the night, so you seem to be doing fine with the increased pressure.

I would get a collar on order, and try going to 18 and see if that kicks it down some more. If you still getting a lot and can take even more pressure try 20 and start the process of getting an appointment with your Doctor

If you were closer to AHI of 5, then I would say try and get used to it and see if it settles down.
I have heard of some needing to go to a higher level machine just to get the higher pressure also offered there, just not very often.
You either need the higher pressure or something else is going on. The good news is that at least you are sleeping while trying to get this under control.

I looked back at all of your plots and in this latest you did at least get some time with zero flow limits, the pressure is doing something.

at a guess, you need pressure upper limit to be in the neighborhood of 20. You may require a bilevel to get more. You stay pegged at the ceiling, and your leaks seem negligible... some things that may or may not reduce pressure requirement are side sleeping and head elevation, and at your pressure, I would try those; but in the end you just might need the next level machine.

Tongue Suck Technique for prevention of mouth breathing:
  • Place your tongue behind your front teeth on the roof of your mouth
  • let your tongue fill the space between the upper molars
  • gently suck to form a light vacuum

Practising during the day can help you to keep it at night

I really do appreciate everyone's input. I'm puzzled at this point. I think I'm going to go with this for now:

Before I started all of this, I was starting to feel some benefit with the single digit AHIs, albeit >5. My urge to nap in the evening had diminished, and I felt better at work. The last few days that hasn't been true, and I've succumbed to the recliner monster for hour long naps. I've woken up with headaches that have lingered all day. So I don't see the point of continuing to crank up the pressure when I had much better numbers at lower pressure. Something else has to be going on. I'm going to back off the pressure, and go talk to my doctor. He's not a sleep specialist but he's good.

How about that! Still not considered adequately treated, but It may improve with time...we've seen it before. Please continue to follow-up so we know what your doctor decides to do on this. You stumped the chumps here. Best of luck moving forward!

It could be possible that you are less prone to having mask leaks at 10 cm and therefore sleep on your side more. I'm totally reaching here, trying to think of an explanation. Smile

You could spend a few hundred dollars for some of that fancy tech gear like the Withings Aura that records your sleep position.
