New to Bilevel and lots of CAs

New to Bilevel and lots of CAs
I've had some success with APAP in treating my sleep apnea/UARS, but even at a higher pressure (18.8cm H2O) i'm still getting hypopneas and RERAs. My sleep doc mentioned I may need to move to a Bilevel, but I can't get an appointment with him for several more weeks so I've rented a Bilevel to see if I can get this right while I wait for him (I did most of the config on my APAP rather than him, so I'm not hopeful he'll be able to help, but anyway)

I've kept my IPAP at 18.8 and I've tried EPAP of 11.8, 12.8 and 13.8 (PS of 7, 6 & 5). At PS 7 and 6 I'm getting a lot of CA events and at PS 5 I'm getting about the same as I had with EPR 3 on my APAP, but I'm still getting a lot of disordered breathing.

I'm reading lots on Bilevel today to see where I should focus my adjustments, but I was wondering if anyone can help point me in the right direction. I'm considering getting an appt with Jason in the meantime to help me get this right.

I've loaded my data into OSCAR and attached a photo.

RE: New to Bilevel and lots of CAs
Welcome to Apnea Board,

Others better at this will comment. A few things about your Central Apnea, did you have a sleep study, and do you have the detailed results? The focus is were the Centrals in the test or not?

Edits I'd consider would be reduce PS down from 6 to 3 or 4. The goal is to address reducing these CA. Second possible edit is the cycle, reducing it from high to medium. I don't recall very many needing to increase the cycle setting.

Your choice of high trigger is a good idea, given the CA. Here again is another option, maybe try very high trigger. Again to address the Centrals.

Depending on the results of the possible edits and whether they affect your CA will determine actions later. If you happen to be a person that's in a select few that the Central Apnea is predominant, then the current VAuto wouldn't be the correct device. That best Central Apnea machine is the ResMed AirCurve 10 ASV. It's not at all certain you need that, as this is yet to be proven.

RE: New to Bilevel and lots of CAs
Thank you for the advice and the reminder that I was missing key information. I did not have any CAs in my sleep study. I had been on APAP for 3 months and still had CAs, but it was approx 10 a night.
