The reason naproxin (we call them "nappers" here ;-)) isn't helping is that it does something a little different than diclofenac, binding to different receptors, etc. Votaren or Olfen, which are more or less diclofenac, is the med of choice here for your type of pain. It is exactly what I would have prescribed. Get the injection anyway - it will lubricate the joint, lower the inflammation of the bursae and act as a cushion for a while, which will help healing.

RE: AirFit P10
Ok, I will factor that into the decision. Thx.
I am 80% better than just 24 hours ago. Not well, but vastly improved. Now it is mostly just very annoying rather than seriously debilitating and painful. My Rolfer helped quite a bit. The only reason that I expected the naproxin to help was that it does help when my knees flair up to their worst.

The Large size is indeed a big pillow but I think it is a better fit, I feel I don't have to worry about little leaks creeping in at the sides as much. The two nights I have used it my leaks have been 8 and 9 in the 95% so I am happy with that. I will definitely take on board what you said about removing gently as my nose is very tender since changing to Large. Glad to hear you are feeling some improvement and hopefully you will not need injection but if the pain is too much, have it, why suffer any longer than you have to. Thanks for explanation of Rolfer, I have never heard the word before. I have heard of some of the similar practices and know they can produce excellent results, so good luck to you and your Rolfing.

RE: AirFit P10
I like the P10 best of the 3 masks I've tried in my 3 1/2 months on CPAP. I started out with the medium as recommended by the RT and I finally got him to give me the large size (he doles them out rather than giving me the set of 3 that comes with the mask). The large size provides me with better air flow. The last two nights my AHI has been 0.0! I, too, have to wear a chinstrap so I put it on first with the P10 over it. Works great! Now if I could just get the aerophagia under better control!

Yes. I had a doggie awhile back that was having a lot of pain. I took him to the vet and he got an injection. Now he just sits quietly in his little baggie.

Different problem, actually, and naproxen is the drug of choice for knee inflammation, unless it is of the bursae. What it is doing there is reducing the inflammation along the joint and ligament structure - it is a cox 1 and 2 inhibitor, sort of like a super powerful aspirin, with the same dangers of bleeding, etc. Diclofenac is a sodium channel blocker and a powerful anti-prostaglandin and in fact far more powerful than naproxen in same levels of dosage, and better tolerated.
RE: AirFit P10
DocWils: I would love a little more on the Diclofenac working in the Hip vs. the naproxin in the knee.
I am 80% better but it is pretty obvious that the diclofenac is masking the problem since when the time runs out my pain returns substantially and it disappears largely within an hour of the next dose (might ask doc if I can take 3 instead of 2 daily.) BACK TO THE P10: I know it might sound DUMB but that simple trick of taking the darn pillows out CAREFULLY instead of just pulling them out IMMEDIATELY made a large difference for my nose. Think "jerking the bandaid off vs. pulling it slowly" EXCEPT instead of getting you through the pain quickly, the careful removal of the nasal pillows avoids the irritation almost entirely. Seriously, I have been advocating "LEAVE THE MASK ALONE", "Don't FIDDLE with it", and if you do this the darn thing will be almost glued in there on waking. Pull it out carefully with side ways or alternate left-right pressure instead of just pulling it down and out. My nose knows the difference. Your nose will thank you. I think I will go post my "Success Story" (finally) as it has been 90 days with 100% compliance.

RE: AirFit P10
In the few days I have been gently taking pillows from nose I have definitely noticed difference and my poor old nose has ceased to be sore.
Thank you for that vital tip Herb. I am also pleased I have changed to the Large pillows as they are a much better fit, mostly from as soon as I put them in I don't have to touch them. My leaks are consistently lower now, very low, and nice low AHI'S. I think I have finally found my mask and size of pillows that suit me best. I have tried the Swift FX, the Swift FX Nano and now the Airfit P10, all in about ten weeks, so I am quite happy with my outcome. Thanks Herb for guiding me towards the Airfit P10. Now to see my sleep specialist this coming Monday and hopefully he will have some answers for me regarding my extreme daytime tiredness. PS. Herb, hope your pain is easing...

I'm pretty sure I am a size medium and 1/2. At least in the right nostril. Either that, or I have a hole in my nose.... Every night I put on the mask, turn on the machine. Solid as a rock.. No leaks. Lay down, solid as a rock. No leaks. Sit very still a minute? Psssssssss... Leak. Right side. Fix it, solid as a rock, no leaks. Wait a minute, psssssssssss.... leak. So replace the pillows, how hard could it be? Solid, wait, pssssssssssssss..... leak. So the message is clear: the right side of my nose has a hole in it. Overall my leak numbers aren't too bad, but a psssssssssssssssss in the middle of the night can be deafening. But, carrying on, tonight I think I'll start with a medium, then at first psssssss go to a large. |
