Wow, I feel sorry for you. That's a trainwreck of something that's supposed to be PAP therapy. Or a new twist on the old Etch-A-Sketch.

I can't be certain, but it looks like every few breaths as seen at this zoom level you get a CA. Why? Unfortunate but that I don't know. This is probably just Captain Obvious though.

Had we discussed or considered straight CPAP setup with very little variation until the ASV? Then again, that may not help either.

That graph is an image of a patient being treated using an inappropriate therapy device when he clearly needs ASV.  I think I previously used the term "malpractice", and that still seems appropriate. The image is a large cluster of Cheyne-Stokes Respiration and should have been flagged as such.  You need to be on the phone to your doctor's office today. If he won't treat you competently, you need to find someone that will.

Listen, we need to mitigate this travesty by moving back to the fixed pressure and turn off EPR as we did with the previous machine.

The technican came on my demand and sugested fixed prssure at 9.
How can we than see if it is efficient or still prove that I need ESV?

My friend, after this much time, there is no longer any question. Demand the machine or request a clinical titration including ASV. They are destroying your health and getting nowhere. I think the way to approach this is to ask "what is the procedure for approval of ASV?" Do not let them change the subject or evade a real answer. You want to know how they are going to get ASV approved and nothing else. That is what needs to happen right away. If they do not answer you directly and logically, they cannot help you and you must change health care professionals.

WHY are you dealing with a technician? You need a physician. Unless I am misunderstanding how things work there, the technicians job is to make whatever you have been prescribed work. They have no authority to change the prescription. You need to get away from the tech and get to the person authorized to change your therapy and make an appropriate prescription.

Let me repeat SleepRiders comments.

You need an ASV

An important consideration for possible parrallel treatment.  Do you have any heart issues.  This pattern has a possible cause of you having CHF, Congestive Heart Failure.  While unlikely, it is important to eliminate.

A typical cause is your bodies CO2 levels are fluctuating above and below your bodies apneic threshold.  The up and down in the flow rate chart is a volume change.

An ASV works by maintaining volume, thus eliminating this pattern and your Central apneas.  It is the ONLY machine that does this.

Your machine maintains pressure which doesn't help at all.

Follow SRs suggestion to as much as possible avoid this

I agree with you all. 
You guys and gals are the specialists by personal trial and error. I have great respect for such experience and knowledge.
The thing is that I have no real authority to over(sleep)ride a specialist, even with all of you as supporters.
Sleeprider, you have found the diplomatic solution. I will send an email directly to the doctor asking -the suggested questions. How do I let him realize that ASV is the correct solution? I must have an argument since I ask for a special option which may seem a bit mrknowitall ?

Anyway, it may turn out to be an informative thread for the forum users... Big Grin
