Are there standard fluctuations in AHI readings?
I am wondering if differences in AHI readings from 12.xx to 39.xx are reasonable to expect from cap therapy from night to night?

Simply put, no. A variation from 0.1 to 2.5 might be not totally unexpected. A baseline of 12 is really, really high. Untreated high. 39 is off the charts for a treated AHI.
Post some details and sleepyhead screen shots for help.

With a pressure of 14 and you are still facing up to 39 AHI, you may need more than an APAP to treat you. Posting your Sleephead to see what type of apnea events you have will help.

It depends on what machine you are using, your settings, and the types of apnea. You are having.
Acceptable? He'll no. Post your daily charts and your sleep study results and I'm sure we can guide you into better more consistent therapy. Fred

Sounds like complex or central apnea. You most likely need a change to ASV. Do you have decent insurance?
