Hrmm CPAP place called and now they don't want to give me the ASV because the prescription didn't come from the sleep doc.. I said I tried to book with him but he didn't want to see me, so went to my doctor who studied the Titration guide before writing the prescription as he didn't want me to wait until next year to get sorted.

They told me to switch to fixed pressure because that will make my CA go away (I said I was already on fixed and it didn't go away at all).... (I mean I had CA in my sleep study I don't think it's going anyway) but to shove it back at them I will do what they asked then complain louder.

Some don't get it. Sometimes these CA are too stubborn to allow the Avoid Treatment to work. Quack quack.

The frustrating thing is they didn't even bother to look at my sleep study report at all. The first day I met with them they just gave it back to me without taking a copy or looking at it. During the "home titration" I became frustrated that they didn't seem to do anything every time I went in, and literally had to force them to change settings etc.

I am also curious if the sleep doctor even looks at the reports properly or just rely on the technician. Both you and bonjour picked up on the idiopathic central apnea which was never mentioned to me at all when I met the sleep doc. He just kind of smiled at me said I have sleep apnea and I need a machine here is the prescription I will see you later.

I am curious what the CPAP shop thinks setting me to fixed pressure will do? I assume they must have a theory right? Although they never saw the CA in my original sleep study so maybe they are assuming it is treatment based CA?

Sounds like the sleep doctors are equally incompetent around the world. The lesson is, don't use a specialist, get the doctor responsible for your primary health involved.

Its wildly infuriating..

How long would it generally take to feel better on something like ASV if it is the central’s that are causing me to still be so exhausted and they are resolved? It feels so close but so far to knowing if this will work or not. If I was to get ASV and all my central and obstructive are gone and I still feel like rubbish then it’s obviously something else but I cannot think what it would be at all at this stage other than potentially this (and the fact they are waking me up).

I would be more than happy to go back on APAP (or Bipap if it can get rid of the aerophagia using PS) and accept central’s until/if they become a major issue.

It appears stupidity reigns over sleep specialists all around the globe. Work with the doctor that seemed most inclined to work with you on the CA, which it sounds like it is not the sleep doctor. I'd say all sleep doctors I personally have dealt with have a mindset to treat obstructive apnea and nothing else can possibly exist to poor sleep, not even CA. My symptom and complaint list that includes CA has brought about some claims I wasn't doing something right, it was a mental issue, it must be a bad machine.

A sincere piece of advice is do not work with a doctor that dismisses CA as it is your problem or a machine issue. If they will not take you seriously then fire them immediately. Work with that doc that had the most promise of assisting you with CA and skip the rest of those dopes. And if the gaggle of dopes include those at your supplier, change suppliers as well if you have that as an option. I find success is best found with only working with those that will help and will listen to your issues. If they aren't willing to help, they do not deserve your business.

Almost seems like a waste, he (the sleep doc) seems to be highly qualified I hope it is just something he overlooked? It almost feels like he didn’t write the reports or look at anything other than the red part that said “severe obstructive sleep apnea CPAP recommended”.

I am considering going to another supplier and I asked them to contact the sleep doc and try to sort it out. They asked me if I had private health insurance I said no I have money.. hopefully they got the point and hurry it along.

As for timing on how soon ASV makes things better, 1. the charts get better looking pretty much instantly. 2. you may have a small learning curve to deal with the ASV breathing patterns but it's not a drastic thing in my own usage. It does like to blow hard at times in the beginning of the therapy lifetime, meaning right at the beginning night or two of therapy. It seems to misread the drifting off to sleep breath pattern change and decides to say hi with a strong blow. Just blow back through the mask and greetings are over.

I would think that using near default/titration pressure settings would probably yield better sleep even night one.
