2 years on and still fighting with machine
Hi, would really appreciate some advice on setting up Resmed Airsense 10 Autoset. I am using the resmed F30 full face mask. Tried the F20, both silicone and foam but AHI's were consiatently high.

The sleep consultant recommended a max pressure of 18 cm H2O.

I do get a lot of leaks and haven't yet worked out how to adjust the mask properly. One night it will have minimal leaks and the next, without any changes to the strap positions, it will leak like a sieve.

I have tried following the mask setyup advice on this forum but with little success. Still waking up numerous times during the night waking up non-refreshed.

Any guidance would be very much appreciated.

Your sleep is extremely fragmented as the CPAP increases pressure in response to clusters of events. We can deal with that by doing two things. You need a minimum pressure of 9.0 and reset your EPR (exhale pressure relief) to 3. Your EPR is currently at 1, and a higher EPR will reduce flow limitations and help with the very high pressure variation. The clusters of events suggest you are obstructing due to positional issues. This is where your cervical spine (neck) bends and obstructs your airway. Since that causes events and drives pressure higher, you are arousing and removing the mask. There are a number of ways to address this, but the soft cervical collar is nearly always effective. Read these two wiki articles. Note the chart examples of other members that shows the characteristic obstructive clusters.
