[CPAP] First night on APAP and AHI was 19

First night on APAP and AHI was 19
I have been recently diagnosed with OSA. My AHI during sleep study was 62. Yesterday was my first night using Resmed Airsense 10 Autoset. During sleep study with CPAP, my AHI dropped to 0 but yesterday OSCAR showed me that my AHI was 19. I thought it is suppose to be less than 5 with CPAP. Also the machine pressure rate is between 4-20 but the average pressure rate was 16 last night. How do I decrease AHI to less than 10 atleast.

You're right to think your therapy is not optimized. You have way too many obstructive events and a lot of flow limitation and snores, and you are having events even with your EPAP at 17. I hope one of the experts will swing by to comment. In the meantime, a couple of questions:

Do you think you were asleep between 5:45 and 6:30? I'm guessing you were awake, but it'd be good to know.

Also, what are your sleep positions? Sides, back, stomach? Do you use a high pillow or a stack of pillows? Do you know whether you tend to tuck your chin toward your chest?

I'm sure this is positional apnea.  That is when you cut off your own airway by the position you are sleeping.  The way many people have helped this is with a collar - see my link to it at the bottom of the post. (collar)

There is no way your machine can stop this type of apnea the only thing you can do is to figure out how you are cutting off your own airway and stop doing it.  If you go to the link I provided you can see some examples of charts where there is a before and after the use of a collar.

Many times people chin tuck because of sleeping on their back or having to high of a pillow.  But yours is bad enough that it may take more than changing the pillow or side sleeping and that is where the collar come in.

This is about as clear cut of a case of positional apnea as I've seen in a while. Very easy to resolve. Please read these Wiki articles. Note how the example "before" charts in these articles look just yours. We're going to make you look like the "after" charts very soon. Please increase minimum pressure to 9.0 and EPR at 3.
