Help me optimize my therapy
This is my first post here  Smile . I feel that my OSA is treated pretty well, my untreated AHI was 15.8. With APAP therapy, my AHI is rarely over 3-5 but I'd still like to optimize my therapy. I just received a new APAP machine, the AirSense 10 Autoset. I did have a multitude of events last night, so any feedback to address them would be appreciated.

Please show the entire night, not just zoomed in spots. If anyone wants to see something in particular they will ask for a zoomed in image. You have the correct charts, just not the overview we use to begin the process.

Generally we like folks to start with something to use as a baseline. Try setting your pressure range to 7-15 EPR 2. Once you have a few nights on that post a chart and we can fine-tune from there.

You have very high flow limitation which is a partial obstruction of the airway. We can greatly decrease the flow limits using EPR (exhale pressure relief), and that will result in better comfort, less pressure fluctuation and better sleep. You will need a minimum pressure of 8.0, Maximum pressure of 13.0 and EPR On Full-Time at Setting 3. These settings are in the clinical settings of your CPAP. The higher minimum pressure is used to accommodate the use of a 3-cm pressure drop during exhale, resulting in a minimum pressure of 8.0/5.0 (inhale/exhale). The maximum pressure will be capped at 13/10, which will be adequate with EPR based on your current results, and I expect you will stay at the lower range of pressure. Give it a try and post new results tomorrow after trying these settings.

Flow limits went from 0.12 to 0.03 along with a normal inspiration/expiration ratio and better tidal volume and minute vent. Feel better?
