In defence of my CPAP clinic at the Royal Papworth, this last year, and now with with a record number of sick and dying because of all the plonkers who ignored the rules over Xmas, they have all been seconded to the care of covid patients. So my CPAP Ward is completely taken over with covid patients. It has been difficult to get hold of anyone from CPAP and to be honest because of the situation I prefer not to anyway. I will let them have the card from my new machine when I go in again.I always go in as an inpatient never as a day patient so they will have plenty of time to read the card. I have no idea when this is likely to happen. I think we will be lucky to be through with this by the end of this year. I have felt all along once it is all sunk in that 2021 was not going to be that different. However vaccinations are taking place and John and I will be having ours shortly.

My reading was very good however I could only eat soft food yesterday and at the time all I could think of was ice cream which was not the best choice for me as me and carbs do not get on. However, As usual it's zonked me out and I slept very well. I do know the scientific reason as to why carbs do this to me but how the hell it improves my breathing I do not know.

I started writing this over an hour ago but I fell asleep and I am now going back up to bed. I do not think this has anything to do with sleep apnoea but just to do with physical stuff I've been through this week and the fact that it is 0° with ice all over the place and all of my joints hurt. And of course I am hungover from the carb OD.

Still looks fine. Sometime in the future days, I want to see you try EPR at 3 and see if that is more comfortable and knocks down the flow limits a bit. No rush to try it, and it's good to establish a good baseline. The Resmed Autoset is responding predictably, and is very sensitive to flow limitation which causes it to increase pressure. That is exactly what you want, because flow limit is an early warning that obstruction is beginning. This is why I described the Resmed as preventive rather than reactive. Clearly this is working better for you. I won't get between you and your ice cream; I'm old enough to know better.

I am confused. The first Remed reading you suggested I could put my lower limit up and my EPR down to 2. I did both of those and that is what you see in the second chart. A min of 11 and an EPR of 3.

I have meant to ask you why in the left hand box, under the AHI number, there are far fewer lines than with the Dreamstation. No RERA, no Variable Breathing, and more. Why is that? 

As this machine came to me unmolested, I was left to put all the settings in myself. That is precisely what I wanted. Before they sent it to me I did receive a phone call asking me what settings I had been given. I explained that I wanted it to come without any settings, that I knew how to do it. As the machine quite different from the dream station there are settings that I didn't really know what they were. I remember one of them gave me a choice between plus and something else and I chose plus. However everything else seems pretty straightforward to me.

You don't need to say anything about the ice cream because I know it is not a good choice at all. I am quite concerned as to what I am going to do once my teeth are all gone and I am waiting for a full heal before I have my first set of dentures. My neighbour has had this pain and her husband blended all her food for her and soft high carb foods which I cannot do. I eat a lot of elephant yam and in the rice shaped form it is very easy to eat I can buy jars of low-carb pasta sauces to flavour it with and I am fairly confident that I can eat the tofu either by squashing it between my tongue and the roof of my mouth or by bending it and adding it to the elephant yam. Are usually have a stirfry every day with the tofu and elephant yam but with the addition of vegetables. John prepared it all for me and then I cook it. It is then that I add all the spices.

I had a nap this afternoon which was rather disturbed because of pain in my mouth and this resulted in a reading of 6.8. I had problems with the mask, tho I don't usually, and had a high leak rate. In fact the nap was divided into two. The reading for 1:40 was 4.8 but large leak of 27%. The second half was 10.31 for 1:04 and all were CA.  NO leaks. 

I was surprised I had slept that much as I felt i hadn't because I it felt like I had kept waking due to my jaw. 

Sorry, I have written this over a period of hours due to disassociating. It is very inconvenient. I am not writing anymore. I hope what is here makes sense.

I'm sorry, I forgot the previous discussion with EPR. Just keep the same thing going for settings.

There are more events on the Dreamstation because you had more events to show. If you start to record RERA and some other events, Oscar will show those, but for now you are treated such that those are not detected. The Resmed does not FL, Snores or VS2 as events, but provides graphs of flow limits and snores, and useful statistics. Variable Breathing is not used in Resmed, but it will show CSR (Cheyne-Stokes Respiration) if significant periodic breathing occurs. Apnea and Hypopnea are the same.

In the settings, there is a choice between Essentials On or Plus. This affects only the on-screen display, and I recommend Plus.

Find a dental office that specializes in Implant Dentistry. You can have your teeth back prettier and better than ever, and it will save your jaw bone from deterioration. I'm surprised no one has discussed this with you. Implants are far more practical, and natural than dentures.

Maybe time for a second opinion on the path of your dentist or oral surgeon. Your situation seems radical, even for an Englishman.

ME: This what I did when I first set the machine up. I am glad this machine shows less for the reasons you give. Although I do wonder why on the DS the card showed events that were mostly always 0.00. However, I was getting a lot of periodic breathing on the DS, just not nightly. Why oh why do so many Americans think we are so backward here? Another assumption, which, frankly is offensive, but I can't be bothered getting annoyed at such silliness.

ME: you really must learn not to assume. I've got used to you now so I find it amusing rather than annoying.

To have my my teeth removed all at once would have required a GA which is too risky for me. My teeth cannot be saved. They are rotten. Years of dry mouth and meds. Plus for 5.5yrs, as I briefly explained, I was a completely different person. I did not go out, I only ate certain foods, one at a time. For about 3 yrs it was Tesco Finest Granola Squares and nothing else, then it was Young's Jumbo Scampi, oven cooked, until finally it was black grapes. Between May 2014 and Sept7th 2019 I may as well have been in coma for all that I recall. I only know about the food because John told me as have the staff that would bring the food to my car late at night as John was still working in London then and only home at weekends. I have photographs of me during that period which prove what I have been told. The most surprising is a photograph of myself smiling showing perfectly okay teeth. Then a few months later, my teeth are black. I was back on this planet by then so I bought an electric toothbrush and some expensive paste. First time I used it, I lost several teeth.

I went to the dentist. They x-rayed me, told me none of my teeth could be saved and that I needed them all out. However, I needed it done in hospital as they would not do it given my health issues. To be honest, doing at her surgery, in my chair would not have been easy. The hospital is much larger and I have been able to use my own chair for everything but the X-Rays so far. (I have a Quantum 4-Front, custom made.)

In due course I was seen at the hospital by a lovely woman. She agreed with the referring dentist. Much to my surprise, she said she would do all of the work for free. Including dentures. With the referring Dentist that was going to cost me £4000. The referring dentist possibly regrets her decision to refer me but I do not. Not because of the money, although that is very welcome, but because Claudia has not caused me pain once, understands CPTSD, altho with her being female, it isn't an issue anyway,  

Now the type of teeth you are talking about, implants, cost anywhere from 9,000-25,000 depending on type.

Assuming I wanted to spend that amount of money on implants, I don't want them. I have looked into it well enough and the very idea of having bolts screwed into my jaw bones horrifies me. I can imagine how easily that could cause long term problems and pain ESPECIALLY in someone like me who has a neurological pain problem. 

I get to choose the colour teeth I want. There those who want nicotine stained teeth so they remain the same person!!!
I just said 'normal' and what I said next is the most common request, said to me by both dentists, 'please just not American teeth.'
I am sure you realise from what I have written that dentistry is no longer free (except in situations such as mine as I have special needs and diseases that automatically qualify me for free everything. ). I cannot remember when it was split off from the NHS. The Govt will say it is still part of the NHS but finding an NHS Dentist is a nightmare and even then, it is not free, just reduced.) Also, it isn't free anyway. We all pay health insurance through our taxes. Pay slips will list your salary, the monthly pay, the tax you paid and NI you paid. The NI part is health insurance. Successive govts over many years now have mishandled the spending of the NI money and have used it for purposes other than healthcare.

Glad you're getting used to my ignorance, but you could have headed it off by simply explaining you are heading to implants. Don't assume that I assume things are "backward" in the UK, but you are going through some traumatic issues, and not suggested anything concerning the end-game. It's funny, and obviously we are mutually amused. I'll assume you are not opting for the nicotine stains as your best option. Smile I hope John is the one with a sense of humour in the family.

ME: I am told I have warped sense of humour and find humour in the worst of life. I fail tho to see the humour in your joke above. Maybe the combo or pain/feeling ill and drugs are clouding my mind.

So the assumption you had that I haven't been to a Dentist that specializes in implants was meant to be funny?

And the assumption no one has discussed my options with me was also meant to be funny? Perhaps my age is dulling my game. Most who know me think I am funny. 'oh, him. He's so funny. Most people would have killed themselves.'

I know our languages differ but surely not to the extent that you think my words translate to  'I am heading for implants'? Maybe I wasn't clear enough in saying would not have them and the reason why. 

My neighbour, Raff, in this mid 70's has them and he no issues with them. He doesn't even use fixadent. 

I was very surprised when I discovered what implants were. I had thought that each tooth was screwed in separately. As you know, this is not the case. The screw bolts into bottom and upper jaw and the dentures fix on to them. If one ignores the bolts in the bone, it seems like a good idea. 

I don't know why you'd think I would start off by saying I don't them. How could I anticipate that you'd just assume I knew nothing about them and the professionals I have seen wouldn't have mentioned them. That is why I have the impression that along with thinking we are socialist/communist country, we are also backward, to a lot of Americans except those who travel or who live here. There is an excellent young American on You Tube whose channel is devoted to teaching Americans about our 'wonderful' NHS in his words and other American misconceptions.

I like Americans, generally speaking. I fulfilled a dream in April 97. I took a plane to Logan, got in a Car and drove to the other side of main, on the NB border, drove north, stayed in Presque Isle, from them onto Quebec City, Monreal, Ottawa, Toronto, Niagara, New York State, Connecticut, Vermont, and the last few days I stayed in NH, SWest Lebanon and travelled around using my hotel as my base. It was the most beautiful and the most scary place I have been to. I found out how different our language is.  I never got used to seeing men walking around carrying, mainly in Maine. I didn't like to see the police with them either. 

Upon leaving Boston, an example of my ignorance you will enjoy, I saw road workers being protected by armed men. I thought to myself that crime must be terrible here is the road workers needed armed guards. Upon mentioning it to someone later, I was told they were armed Police and the road workers were prisoners. Oh!

I drove 3,800 in 13 days. I loved every scary thrilling minute of it despite the physical agonies I had cos I took no meds, too scared to.  I have no regrets. It is probably the only stand out good moment in my life other than meeting John and later marrying him. I collect the few good memories I have and treasure them. At least I have some. At least I am still here. 

Oh, and from a medical pov implants are not a good idea.

I must retire. It's already a bit late for me at 01:20. During the last lockdown our bedtimes and rising times went to pot. Once I was back swimming I was back to midnight to 8-9am. 

I just remembered I didn't address your comment about the end game. Well, we know the end game, or at least I do. Maybe before I have time to get used to the falsies. Maybe in 25 yrs time. Maybe tonight. WTF knows? 

If I let myself think about it, I could find myself rather angry that in the last 20 years with all the investigations, with all the serious issues they found, not one of the medics had SA cross their mind.

Here is my Resmed report for last night. I am not a happy bunny. The Phillips Auto gave me high AHI and this gives me higher ones, except for the high carb night. The consistent low AHI were produced my using the DS CPAP set to Auto-Trial. 

I forgot to say that the machine gave a lower AHI, 8. something. It then said AHI 4.? and I am sure under that it said Central Something 4.? added together they totaled the same as the first number. I can't, as far as I know, return to that screen cos I went 5 mins over whatever their day is and thus lost it. As you can see from the chart, it doesn't show tho it does show the 5mins over at AHI0.00. I have bothered to include that in  this chart.
