Question for the group. Would I need another script from my DR, to upgrade my CPAP machine? Or, can I just call my DME office and ask for the upgrade?

You will need a different script from the doc. He/She needs to write it differently than the original. The prescription needs to state the exact brand and model (Resmed AirSense 10 AutoSet), along with the verbiage, "Dispense as written".

The Resmed Airsense 10 CPAP and Autoset are both simply CPAP machines and covered by the same insurance code, HCPCS E0601. Technically you do not need a different script to make this exchange, however if your doctor agrees you will benefit from being proactively involved in your therapy, and he also wants to know more precisely the effectiveness of therapy, then he can modify the script to "dispense as written" and specify a particular machine. You can first make a request from the DME, and there is a chance they will provide a replacement. We have seen it happen. It is also likely that the supplier only sees this as a loss and your current machine meets the requirements of your prescription, and will deny a change. So maybe ask first, then if denied, get a modified script. Your prescription is also valid for any private purchase through any online supplier or a private party. I have often seen lightly used Resmed Airsense 10 Autoset machines available for sale on Craigslist, Facebook Marketplace, Offerup, DotMed and others. These generally range widely in asking price from very affordable to grossly overpriced. It helps to know what is fair. For reference, there is a brand new Resmed Airsense 10 Autoset listed in the South Florida Craigslist for $495 and and others without specified hours ranging from $225 to $425. Your filters, humidifier chamber, tubing and mask can all be used with an Autoset.

Agreed, it doesn't hurt to ask. Way off topic, but I've got an insurance paid-for power rehab chair that was tagged at $28,500 as it's a custom chair. How did I get that? I asked my docs about it. That's where it started. The question never asked is guaranteed to never be answered.

Hello all I'm back. Still working on upgrading my CPAP device, but in the meantime here's an update on my CPAP experience and hoping for some one for help. My Aerophagia dealings continue. Since my last post, I have tried all the suggestions provided to me, excluding the CPAP upgrade. (Which sounds like the only option at this point).
Here's what I have tried so far, and mind you I'm getting anywhere from 5hrs 30min to 6hrs 30mins of continual mask use when I sleep: First up, a Chin strap; Took me couple of nights to adapt to this device. I adapted ok, Aerophagia continued. Second up, Cervical collar; again took me a couple of nights to adapt, I adapted, again Aerophagia symptoms persisted. Third up, Chin strap plus cervical collar: Aerophagia symptoms return. Fourth up, taping; I have started taping this week, and surprisingly I adapted after a few minutes. ZERO air is escaping out of my mouth, and to my surprise, Aerophagia symptoms continue even after breathing strictly from my nose!! I remove the mask and tape after 6 hours of use, and get additional 1 hour or so of sleep without my stomach feeling that its digesting a beach ball. I have zero issues using my FM, no air leakage issues. I'm comfortable sleeping with the device, its just this Aerophagia that's driving me nuts!! As I mentioned at the beginning of this post, upgrading machines might be the only solution. I'm all ears. Thank you for reading.

Aerophagia can best be dealt with by using the EPR feature on your CPAP and titrating pressure downward to the threshold where you don't experience discomfort. This means start at your current pressure and turn on EPR full-time to a setting of 3. Try that pressure and note if aerophagia is better, and whether AHI has increased. If you are more comfortable and AHI has not significantly changed, try reducing pressure by 1-cm. You can continue this process until you see AHI rise or you become less comfortable. This is titration by "feel" and it can work, but is a bit frustrating as compared to seeing data and knowing the type and distribution of events.

@ Sleeprider, thank you for the info. My EPR setting has always been set at 3. My next step is to begin adjusting the pressure. My prescribed pressure was 15, and I could not handle that pressure, shortly after, I accessed the Clinical settings and adjusted the pressure to 13.6 and have been at this pressure for 3 weeks. My AHI fluctuates between between .4 to .8. This morning I lowered the pressure to 13 and will begin tracking how this new pressures effects me.

Thanks for clarifying your settings status. The reverse titration should be pretty easy to do and can always be reversed. With your currently low event rate you probably have some room to move closer to a comfortable pressure.
