Can someone look at my 45 day APAP progress data?

Can someone look at my 45 day APAP progress data?
I did my first 45 days of APAP and the company that does the data reports (at the medical supply store) gave me the info. He looked at it and said it looks great and that it worked. I haven’t had much resolution of symptoms, still exhausted, memory problems, irritability etc. and he said that it’s likely from another medical issue.

He is, of course, not the doctor. I’m seeing a specialist next week. Prior to trying the APAP, I was told by both the same guy and some of you on this board that APAP would likely not work for my centrals and that I would likely benefit from ASV instead. I’m attaching my initial sleep study data and the data from using it for 45 days. If anyone feels like looking at it and giving me any thoughts before I see my doctor that would be great. I want to arm myself with as much info as possible.

They did you a disservice by leaving the machine on 'default' 4-20. You still have an AHI of 7.1. It appears this is all from hypopneas and obstructive apneas. If you were to start off somewhere closer to the 11.5cmH2O pressure it may be beneficial.

The iBreeze isn't listed as compatible with OSCAR, unless someone knows different. But I think that you may be able to download the manufacturer's software.

Your AHI remains too high, with most events being obstructive. It is encouraging central events are not being reported, especially since they were present in your diagnostic test.   According to the data, your median pressure was 6.6 and the 95% pressure was 11.5.  Your settings were minimum pressure 4.0, maximum 20.0 and IPR 2 (inspiration pressure reduction).  Based on these results, you should increase the minimum pressure from 4.0 to 9.0 cm-H2O. this will put you in an effective therapy range where your IPR will work well.  Your starting pressure will be 9.0/6.0 (inhale/exhale), and that is a normal CPAP therapy pressure.  The current minimum pressure is ineffective and probably uncomfortable.  The maximum pressure can be limited to 14 cm based on your results.
Mode: APAP
Minimum Pressure: 9.0
Maximum Pressure 14.0
IPR: 3

You may want to increase IPR to 3 if you find that more comfortable.  I strongly encourage you to increase the minimum pressure. This should get your AHI under 2/hour.  This video shows how to access the provider settings.  We do not offer the manual in our CPAP Setup Manuals, but the video should be helpful. There is a video following this one on the clinical settings that shows other settings and features.

Thank you, this is really great info. I’ll speak to he doctor about it. If he doesn’t want to change anything and thinks my AHI is “good enough”, I’ll be changing it on my own to try. Thanks so much for the video, I would have never figured that out ha.

Thanks, that seems to be the consensus. I will push the issue when I talk to the doctor.

I wish it worked with OSCAR, I would be really interested to see the data. Thanks for the tip about the manufacturer software, I’m definitely going to check that out.
