With the collar and the MAD, I had sessions with FL nearly 0 and other sessions with FL much higher.

I have said the I am OK with keeping things the way they are now, not looking for perfection and/or optimization.

What I would like to know though is why my FL is sometimes good and other times not so good with no change in position and/or equipment.  There surely must be a reason for that.

That might be the subject for a talk with your doctor. I really can't speculate why FL is inconsistent, but there may not be a solution short of surgery. I sure don't want to suggest that is a good idea.
