RE: Cervical Collar - amazing!
Thanks ajack I will try to observe tidal volume.  So many charts and learning interactions between them is a bit hairy for a beginner..

Standing here again hat in hand after scanning the internet for information [nada!]

Last night at about 0200 I again awoke feeling like I was not getting enough/or too warm air.  Seemed like lungs were not filling fully.  This is a common re-occurrence.
The humidity was on 3 and it was turned down to 2.  It is a regular slimline hose (non-heated).
Can see no indications of low or restricted pressures that would wake me, do you?  Beginning to wonder if the Auto mode is somehow involved.  The bevy of OAs following is trying to go back to sleep.  Appreciate any thoughts..

You just had OA's going on when you woke up. That's why you couldn't breath. I don't know how you get any sleep with all the events your suffering through. Hope you get into a ASV soon.

Thanks Walla, but that will never happen (<45%LVEF [ASV excluded]) Sad

Have to make do with what I have!

Can you zoom in on the flow rate graph during all those events? I'd like a closer look at what is going on, but I'd sure hate to experience it first hand. Include Events, Flow Rate, mask pressure, minute vent and resp rate if possible. Also, do you have CHF, or is the LVEF unrelated?

I was hauled in with a M/I in March.  Subsequently wound up with 2 stints.  It is my understanding with clogged arteries you are considered to have CHF.  The result was a lowered LVEF.

If this is not what you want to see please advise. After 0200 I spent a lot of time trying to go back to sleep (OA cluster)

Lets zoom in to about a 4 to 5-minute segment of that. I want to see wave-forms. Let's try for 03:40-03:45

First, those are not likely obstructive. I see diminishing respiration to apnea, followed by a gradual resumption. As you know, I am not in a position, nor do I have expertise to diagnose Cheyne Stokes, but that's what I see. The steady periodic pattern with 30 second apnea, seems to define this. I would encourage you to have this evaluated by a specialist that can accurately determine what is happening here. If those apneas can be broken by pressure support, then your next step is a ST variant with backup rate. You might also consider getting a recording oximeter from Supplier #19 to see how these events are affecting your oxygen saturation. If severe, talk to the doctor about supplemental O2 at 2-3 Lpm. The wrist oximeter CMS-50F looks like a useful and comfortable device that may give you some strong evidence of need when you discuss this with your doctor.

We've done our best here, but this kind of apnea does not arise from something we can coach you on, other than to encourage and support your efforts to obtain appropriate therapy. This is not a problem that changing settings or wearing a collar is going to significantly improve. I don't know what your relationship is with your doctor and specialists, but they need to step up their game or refer you to someone that knows how to address these issues. You can resolve this! There are techniques that can help or mitigate the impacts, but it is outside my knowledge and comfort zone. I hope you understand, that as much as I'd like to help, I just don't have those tools.

I understand SLEEPRIDER.  I very much appreciate any and all help I have received here.

  Thanks Z

PS: I have a CMS59D+ recording oximeter and I will check it but do not think there is a pattern of any reduced SP02.
