Your AHI results are very good, now let's concentrate on the "symptoms are AWFUL. Soul-crippling, functioning-destroying awful."  What's going on?  Your settings are nearly wide open with IPAPmax at 33, and some huge pressure support allowances.  That may give us something to work if you can better describe the side-effects you need help with. I assume we're dealing with aerophagia resulting from high pressure. I have some ideas I think may help.

Currently you have an EPAP range of 5.0-13.0. I would limit to 5.0-7.6.
IPAP from 6.0-33 to 5.0 to 20.0
PS 1-20, change to 0- 12

These are not final, but the idea is to narrow things down for starters. Ultimately results may be a compromise between AHI and comfort.

I was trying to narrow down the ranges but didn't seem to be helpful, so just went completely open to gauge what happened with the pressures.

I actually haven't had issues with aerophagia anymore (since I stopped artificially boosting EPAP like my doc wanted me to, to a ridiculous 13-15 EPAP range). Comfort or compliance not an issue, never has. When I say symptoms are awful I mean the fatigue and related effects, which is not new.

Give it time, but my suggestions were further limiting EPAP range and PS, both being something you knew to do on your own.

WOW Lostie 42.  How did you get all of that data off of Oscar and into a Post. Just back on the Forum after a couple of years away and now have new AirCurve 10 ASV. Want to post new data and let someone analyze it.  Forgot how to Post.

Your flow limits are very high and that is usually combatted by increasing ps and yours is set at 1. I’ll let sleeprider comment he has suggested you post Oscar so I’m sure he will be back. But realize that flow limits OA and H events are ALL obstructive apnea.
Apnea (80-100%) 10 seconds, Hypopnea (50-80%) 10 seconds, Flow Limits (0-50%) not timed  Cervical Collar - Dealing w DME - Chart Organizing
