I do think we'd see an improvement with the Resmed, but as I said in post #2 you may also need a soft cervical collar to resolve positional apnea /alignment issues. http://www.apneaboard.com/forums/Thread-...#pid255570

Do you have a ramp function that you could use to bring pressure up more slowly? I like to start the ramp pressure at about 6-7 cm, whatever feels comfortable while you are awake and trying to go to sleep. 

Your machine is doing the same thing again, and that second expanded chart is a good example. Event after event of OA and it is not increasing pressure, and in fact is reducing it. That chart would be a good example to use to say that this machine is not working for you. Yes, I think a ResMed AirSense 10 AutoSet would be better than that. 

About the only thing you can do with the current machine is increase the minimum pressure again. It may help some.

I agree with sleeprider. I think the Cervical collar will make a huge difference.

WOW! You people are incredible! Here is the first night with cervical collar (2nd night with min 9 pressure) and the AHI is 4.2!!!  I slept the longest yet without waking up. I feel awake and alert. First day not greeted by SleepyHead with "Horrible - see your doctor".

Do you see anything I can do to improve further? Here is are the daily and two closeups.

Linnea, this is a great start at progress, and it makes some other opportunities more clear. First off, since you have some residual hypopnea and flow limitation, I continue to believe the Resmed Airsense 10 autoset will be a big advantage due to the ability to set EPR in a way that reduces hypopnea, and that is not something I have been able to accomplish with the Philips machines. Mostly, I think the OA will continue to decline with use of the cervical collar. The OA cluster you highlighted does not appear to be typical of obstruction as there continues to be breaths through the "apnea". It looks a bit more like hypopnea, and could be incomplete airway restriction. I think these events will eventually go away. Let's give the collar a couple more days to work before we make any decisions on pressure.

Something you could do is to simply post your images using the forum image attachment feature. There is a tutorial on that in my signature. It's a lot easier than using Photobucket, and we get a larger, clearer image.

I think as you tweak things it will improve, however, that is not to bad as it is.   Smile

Second night with cervical collar - 1.56 !!! A new sleep record - 5 hours. Does this mean the Dreamstation is working okay or do you still recommend the ResMed?

You have made a great improvement with the collar. If it is not going to cost you a lot of money out of your pocket, I would still recommend the ResMed AutoSet. I think it responds better. But if you can keep numbers like that one night consistently, then it probably is not going to lower AHI much more.

Agree with Ron. The Resmed will kill off the flow limits and hypopnea. These are great results, and I'm sure you will continue to improve on them. I'm not sure which cervical collar you ended up with, but it's really working (no links, name only). It's amazing to me how much improvement many members experience using the collar.
