Hi Moemah,

Sleeprider mentioned my journey being like yours and attached part of the thread. 
I don’t have any clinical advice to give you since I’m new here. I will say the links you’ve been provided are spot on and gave me the knowledge I needed to advocate for myself. 

I was diagnosed at the beginning of September and went through 4 sleep studies back to back. The BiPAP ST being the last study without very good success (AHI 31).  Then I was told there wasn’t anything else to try. So I came here and asked for help, did my research when I could and agreed with those who have commented on your post that I needed an ASV. I just like you was being told opposite advice from the doc office.

I tried the ST for 2 weeks and the data wasn’t any better than my sleep study. So I called the doctor’s office and put my foot down and told them I wanted the ASV now. I also called the DME and said the same. I got lucky and the tech I spoke to agreed I didn’t need the ST. He did his part and contacted my doctor. They did make me do another sleep study on Thursday. Then I had a consult with another sleep doc on Monday and will transfer my care to him. So it is possible to get this going and it won’t months! All of this happened for me in one week. 

I jotted down the highlights from the wiki links and titration protocols that were sent to you and had them on hand when I called both places.  If the doc wants to continue to be evasive, you most definitely can switch your care.  The CPT codes used for the ASV and the ST are the same so another insurance approval shouldn’t be required. You would just need a new script which you could get from a new doc if the current one will not give it to you. 

They’re required by law to give you your medical records. You should be able to get the copies of the sleep studies without an issue.  Regardless, an appointment with another doctor sounds like the priority.  I’m sure glad I found another and wish I had done this from the beginning.

Moemah, there are a number of approaches to advance moving to ASV. The crux is to have a titraton study for ASV scheduled. You are not comfortable on your current therapy, to the point you want to go back to the Vauto or quit using the therapy. The need to address your problem is obvious. It is also clear that your doctor is blocking you and putting his ego first. I don't know where you live, but the truth is you don't even need a specialist any more. Your primary doctor could order this change. If you need another specialist, you have to screen them by asking if they will prescribe ASV for complex apnea. Be very specific you are seeking to change doctors because your doctor will not deviate from ST, even though you need ASV. As I told EmmyLou, both ST and ASV are insurance HCPCS Code E0471. Insurance doesn't even have to approve this swap, you only need the prescription. Your previous sleep studies and titration study will support moving to ASV. You ONLY need a cooperative medical professional that doesn't have his head up his ass.

I would sincerely advocate firing the stupid doc yesterday and replacing with one that will support the ASV script. Oh I am being serious. Fire the duck now. Be sure this new doc is within your insurance in-network list. Take time to note all the symptoms and complaints on your therapy. Include things like the sleep study data, poor sleep, not well rested, etc.

Hopefully this settles it, sending the Dr again asking him if ASV is ever a step in my future he basically told me he is happy I am eager to read but it takes years of practice to know and encouraged me to take university courses Big Grin to learn more so I assume that's his way of saying find another Dr Big Grin
