VPAP lll st -- change to CPAP set-up
I am a long term XPAP user with complex breathing issue.

A friend of mine has been instructed to change a recently supplied VPAP lll ST from 10 - 15 (bipap) to a CPAP at 16+ cm. He has serious breathing issues and sets off his mask leak alarms all the time.

His doctor wants him to go back to a CPAP, since he seemed to have better results with his old F&P 20, but up the pressure to 16+.

The F&P 200 is over 5 years old, due for retirement. I don't trust it to get up to and deliver 16+ cm reliably. He still has this VPAP lll st, which is designed to supply these high pressures with ease.

He has me do any changes & fine tuning for him since I have lots of practice through my own machines. Also, I take the time to explain things to him as I do it. He has serious short term memory damage after his long bouts of severe apnea.

Does anyone have experience with this reconfiguration??

Any suggestions?? I still have to get clinician manual & access instructions but since it is menu driven it shouldn't be too bad..

if wanted to keep using the F&P machine, have the pressure checked
any CPAP supplier can check the pressure for you or get a manometer and you can make your own as shown here

These high tech machines should be able to operate as a CPAP by setting EPAP/IPAP to the same pressure- 16. It's a bilevel machine, but doesn't need to be configured as such Of course you know this. The real question is the machine paid for? If your friend owns the machine keeping it will not be a problem. If not I would try and wrangle a Bilevel machine with an EPAP pressure of 16. Bilevel won't help that much however, as my understanding is that the EPAP should be set to the high pressure that your friend needs, IPAP a bit higher. Many people don't set the EPAP that high. With the clinicians manual you should be able to set the machine up so it's comfortable. If the machine has breathing support I use AUTO. Ramp time may be a feature that is helpful- 16 is a pretty high pressure.

He certainly needs help with his mask fit. As you know a common mistake is tightening the mask too much. Make sure the mask is not too old and cushions replaced monthly for the best fit. I have good success with fitting my mask with the blower on and lightly holding the mask to my face so the cushion can inflate, then attaching the straps. With a properly fitted mask and the correct size I have 0 leaks most nights. The machine should have a mask setting feature which will tell him when the mask is worn correctly.

They switched me from a CPAP at a pressure of 13 cm to a BiPAP set at IPAP pressure of 13 cm and EPAP 8 cm.

Thanks Zimlich --- ALL POINTS duly noted -- preaching to the choir --I wish we could teach all Apnea patients these finer points.

I tried when I was active with Sleep/Wake disorders help group up here, but everything flew way over their heads, even the engineers who should understand these basics of flow & pressure.

I have to use both APAP and a Thornton positioner (mandibular device) to overcome multiple blockages and daily changes. They work great together for me. Took a long time to find the right combo, and an excellent sleep specialist in Toronto.
