Forgot the name of sleep apnea doctor and hospital

Forgot the name of sleep apnea doctor and hospital
In 2011 i was first diagnosed by doctor in richmond, CA. Since then i have moved countless places and i forgot the hospital and doctor name. Only thing i have is the philips remstar auto 550p autopap device. I tried contacting them but their customer support is not helping me with the doctor information or hospital. Is there any way to find out the doctor and ask for old medical records?

If you remember who your GP was back then, he/she may have something in your records.

You can also try checking with the insurance company you used at the time. If they keep records longer than 10 years they may still have a record of the bill the sleep doc submitted.
