GurdBonk Starting

RE: GurdBonk Starting
One thing I would suggest changing: "Essentials:On" means "Minimal Data" will show on your screen. You definitely want 'Plus' - or at least people trying to assist you will! You won't be overwhelmed by the screen display.
You'll have to go into the Clinical Menu to change that: Press the knob once, then press and hold the knob and the Home button at the bottom for about five seconds until the screen changes. Select the Clinical Menu and press the round knob, then scroll down to Essentials, click and rotate to 'Plus' - click again and then scroll back up and click to exit...

RE: GurdBonk Starting
I was wondering waht a gurdbonk was, and I found out it is a prolific poster with a new Aircure 10 Vauto and mixed apnea that lives at relatively high elevation somewhere in the French Alps. (how am I doing so far?). Your proposed starting settings are:
Max IPAP 15
Min EPAP 4
PS 3
Ti Max 2.0s
Ti Min 0.3s
Trigger Med
Cycle Med
Ramp 20min, start epap 4
autostart ON
Essentials Plus/On set to On
Leak Alert On/Off set to Off.

Essentials should be set to Plus, not on
I recommend you start with PS 3.0 based on your history of complex apnea
I recommend you set Trigger Sensitivity to High based on complex apnea.
Turn off Ramp. It should make no difference with a starting pressure of 4.0.

Overall, this looks like a great start with one of the best machines on the market. If any problem arises it will be central apnea events as a result of your mixed apnea diagnosis. In fact your diagnosis is primarily central apnea, and it would not be surprising to me if we are eventually recommending ASV, and almost certainly a lower pressure support. You also have cheyne-stokes respiration according to your diagnostic study. To say the least, i am not optimistic your experiment with the Vauto will be successful. This is a great machine for treating obstructive sleep apnea and hypopnea, but you have central sleep apnea which requires a different approach. It's your choice whether to use this machine or just move to ASV in order to get a full refund on the Vauto. I'm sorry to rain on your parade, but your chances of success without a ventilator with a backup rate is just about 5%.

No rain on my parade, you might just be right.  But I don't have any Chain-stokes events in either study I believe.  And either way, I am better with this machine than none at all.  I will make the changes y'all suggested.   I live in the mountainous Western US BTW.

GurdBonk.  A Nom De Guerre from my misspent youth in HS and College (GaTech) a long time ago.   He was some East German weightlifter we all made fun up because he had the highest recorded anabolic steroid use at one of the Olympics.  I used as as a fake name as a "german exchange" student.   I now have Gerd, so joke is on me.

I pedal mtnbikes and roadbikes around 100 miles a week, play tennis 3x a week, enjoying the heck out of retirement.  But I have to get this apnea under control.   Thanks all.   BTW, I think I should crank up my max IPAP for tonight.....  thoughts?

RE: GurdBonk Starting
You are clearly a very physically fit individual, and that profile is consistent with the central apnea. Just so you know, ASV mean "adaptive servo ventilator" and it is gives CPAP treatment usinga low exhale pressure to maintain the airway, then uses pressure support (an increase in pressure during inhale) to cause a breath or increase the volume of a breath, to maintain your respiratory rate and volume, even if you make no effort. Works great and many members here use them. Your Vauto will similarly give a low exhale pressure to keep yoru airway secure, but it will not trigger higher pressure to cause a breath when you don't spontaneously breathe. It will instead stay at EPAP pressure until you take a breath, then the pressure support will offer a bit of assist.

I'll be interested in seeing your sleep data results, and helping to make it as good as we can, but you're probably going to need ASV before this is over.

I have the titration study copied, but am sitting at my GF's daughter HS first tennis match.  So i will upload the 2nd study in an hour or two.

BTW, I fixed your misspelling in your first sentence ?

Right! I meant fat, but slip of the keyboard. I hope your tennis match goes well, and you take your fat ass home to post more boring data...otherwise, nothing but good luck. Smile
