G'day Mttnkatt. Welcome to Apnea Board.

Starting out on CPAP treatment is very hard for a lot of people. After all you're learning to breathe in a whole different way with a compressor pumping you full of air and a plastic alien strapped to your face! Shock Be reassured though - it does get better.

The settings on your machine (4 - 20) are referred to as "wide open". They won't give you an optimum treatment, but they will allow you to zero in on more appropriate settings. With the Dreamstation it is important to get your settings as close as possible to optimum as the machine tends to respond rather slowly. So I would concur with the advice to increase your minimum pressure to around 9 and see how that goes. As always you should confirm prescription changes with your doctor, but many of us have found that we need to be proactive.

As Walla Walla mentioned, it's possible that some of your apnea is caused by positional issues. Either sleeping on your back, or with your chin tucked down to your chest. You might need to use an extra pillow to stop you rolling on your back, or else get a soft cervical collar to help support your chin. The collar should not be too tight or too tall - it's just a minimal support.

The other thing I notice in your charts is a fair number of clear airway (central) apneas. Did these show up in your initial sleep test? If not, then it's likely these are pressure-induced. In most cases they will go away as your body adapts to the therapy. If not, then we can suggest some further changes later on.

You're off to a rough start, but that's (sadly) not unusual. It will get better and you've got a whole community of fellow apneacs here to help you along.

DB and the others have written all good advice. There are indeed a lot of us who have been helped and have been guided by experienced users here. When you find your balance in what suits you, and with the aid of perhaps soft collars or chin straps or appropriate pillows you will find that you can return to old habits.

By that I mean I am a avowed back sleeper. This causes my chin to droop to my chest when falling asleep to cut off my airway. With a soft collar and appropriate pressures I am now able to return to back sleeping through last nights 9 hours and still obtain AHI zero. This is not often but good enough for me to be pleased and share.

I sleep on my side too but rolling over to my back does not cause events as the soft collar keeps my chin up sufficiently and I also have learnt to keep my mouth shut and breathe through my nose. So, in short, we can learn/unlearn habits to help us sleep/rest better.

Im happy that I got a "not very good" this morning instead of a "horrible" lol

AHI was 8.17 So that gives me hope. Funny but after almost 8 hours of sleep I still couldnt get up this am.

Oh and I tried to change the setting of my start pressure but its locked. Do I have to call someone?

Push the knob to turn on the screen. Then push and hold the knob and ramp button for about 10sec. The screen should change and allow you into the menu.

What we have here, is a pretty good first session (assuming you're sleeping) and a really bad second half. The advise to increase minimum pressure on your Philips Auto is very good. The Dreamstation just does not catch up to events like the Resmed.  Where your pressure starts to get above 14 cm H2O, is where I suspect that positional apnea is playing a role, OR you should actually be on a bilevel machine with higher pressure capability.  A minimum pressure of 9 or 10 cm is probably in order, and you may want to consider the use of a soft cervical collar to minimize the effect of positional apnea (chin-tucks) to reduce physical obstruction, so the air pressure can do its job.  We're looking at mostly obstructive apnea here, and with CPAP, the answer is more pressure, however many members have found that preventing obstructive events with the soft cervical collar.

I was awake the first part.
I'm not able to change my settings.
I tried the 10 second hold and nothing happens still locked. I have a call in to my sleep center

Seriously! Not good to lock it IMO. A demand for better Doc and DME/RT results ought to be mandatory. There are good ones that do help the patient much, but they're too few of them.
