About the leaks: he might want to experiment with a neck collar, which might help to keep his jaw up. The options include a soft cervical collar (available in drugstores), the Releaf collar, and the Dr. Dakota collar. It’s personal which one feels and works best. (If he likes the Dr D but has trouble with fastening it, let me know. There’s a little modification that can help.)
I think some people find that they always need the collar, while others find that at a certain point they can do without it. Again, it’s personal.
If there isn’t a collar that is comfortable for him, the leak wake-ups might be a reasonable trade-off to accept. But I hope he’ll find one he likes.

Thanks for all the help.
A neck pillow is not a viable option.
Here are the last few nights. Trying to fit the mask well before bed doesn't seem to be helping, even though it was not leaking this much before.

To my eye, the changes you’ve made have improved the FLs, and the AHI numbers are excellent. How is he feeling during the day?
