if positional therapy doesn't work, You may need to move to a bipap/bilevel, those numbers are high.

As sleeprider said, I would get the foam collar today and see how you go tonight. for an experiment the next night if it is still rubbish, is to keep you on your side and off your back, by using a backpack full of stuff or whatever is your choice.
you could also try a rolled up towel as a sausage and use it to support your neck, instead of using a pillow. if you feel a collar is not going to work.
but believe it or not, you will get use to all this stuff in a couple of months

For auto-cpap, from machine data or software. You can set the min pressure 1 or 2cm below 95%. Or clinicians commonly use the maximum or 95% pressure for fixed pressure CPAP, this can also be used for min pressure.

You may be using a big soft fluffy pillow currently. This would make you much more likely to tuck your chin and would likely be encouraging those obstructions. My recommendation would be to get a buckwheat hull pillow (on amazon) or a firm memory foam pillow and try that first. The cost will be more than a cheaper cervical collar and may not be enough to do the trick, just to warn you. But it may be worth a shot if you have a hard time tolerating a cervical collar like I do. I switched to a memory foam pillow and it got rid of my own clusters! May be worth a shot for you if you are willing to part with a bit more money.

and I will chime in, as well. 2 things.

1. the exhale pressure is what has to be high enough to hold the airway open. you can change the (lower) exhale pressure by changing the Flex setting or by setting the Mode to CPAP Auto. This can get you a little more pressure if you tolerate it.

2. this night may not be typical, so there may be nights where your pressure may not need to reach these highs.

If this were mine, I think I would target about 13 to 15 as the minimum pressure, especially if not intending to use a cervical collar or something similar.

agreed, a new person bouncing around 20cm all night would be torture. You have to build up to high pressures. starting at 6/15 would be good.

Glad you're here.  The people here are the best.

Just wanted to chime re: soft cervical collar.  I'm not quite two months into PAP treatment, and I've been wearing a cervical collar from practically the get-go.  I am definitely a head-tucker when not wearing the collar, and it has made a big difference.  The other night I thought I would experiment with not wearing it; I couldn't fall asleep because each time I would drift off, I would get a mouth leak-- so, I sat up and put it back on. Voilà.... right to sleep. Seriously, it took me like 2-3 nights to get used to it, and the side benefit is that my neck feels like a million bucks in the morning because it's being aligned all night instead of being in contorted positions.  I'll also concur on the firmer pillow versus a soft, fluffy one.

Give the collar a try.  I think you'll find that it's not that bad.

Tried a different pillow last night. Unfortunately a very uncomfortable one so I did not sleep well because of that and have neck and shoulder pain this morning. But... my numbers seem to be better. Here is the screen shot of last nights data

Tried to do it as an attachment.  Keeps telling me its too big.  If you remove the space between the t and the p in http and paste it, it should work.  I had to do that to get around the "no links" restriction.
