Help reading my Oscar Results

Help reading my Oscar Results
Hi,

I was originally diagnosed with mild Sleep apnea AHI of 5.8 after a lousy sleep study (didn't sleep very well or long) and I tried a tonsillectomy but still felt tired sometimes so I got a Resmed 10 Autoset. I have been playing around with the settings on my own but don't really know what to set the settings at for optimal results. I heard EPR is bad so I turned it off after using for awhile. The lowest my oxygen saturation got during the study was like 86 and my longest time without breathing was right around 50 seconds this was pre tonsilectomy and I did not get another study done afterwards but probably should have. - not really sure if CPAP is totally necessary or what your take on the matter is. I have attatched my last 5 nights of oscar trying different pressures as well as Auto vs CPAP not sure what the right way to go there is either. I noticed when the pressure gets high I swallow a ton of air and its very uncomfortable. I am still having Obstructive Apneas and I am not sure if the increased air is leading to central apneas. Sometimes I have centrals throughout the night and sometimes I do not. Still waking up tired despite pretty low AHI reports - maybe this is as good as it is going to get? Any advice would be greatly appreciated.

Thanks!

RE: Help reading my Oscar Results
Obviously, I don't know your history, nor why you are on an un-relieved straight pressure, and why that pressure is a mere 6 cm H2O.  Have you tried keeping it at 6 cm for the minimum pressure, setting the high at about 12-15 cm, and seeing what happens (I'm assuming your machine can do that...)? I would also try an EPR setting of about 2 cm to start with, but not right away.  Try the settings I suggest for at least two nights, see what happens, and then add EPR of 2 cm.

However, your nightly AHI  are a lot better than even the mild OSA you were diagnosed with.  That may be as good as it gets.  Yet, we don't want ANY obstructions or centrals that arouse you, that last longer than maybe 15 seconds tops, and that don't come stacked-up with three, four, five, of them inside of two or three minutes.  That will put a strain on your heart.

Please be patient and wait for others to offer their opinions.  They'll surely follow.

The clear airway events you show aren't that many and will probably lessen in time.
Did you experience centrals on your sleep study? You said mild apnea, so I'm guessing no.

Are you able to post a chart or two when you actually used the auto setting?
And at what setting did you experience Aerophagia? This is important to know.

I think you would do much better with the Auto mode with some EPR. But seeing a couple charts when you used Auto mode and knowing at what pressure you were swallowing air will help us to advise.

Thanks for your reply. I did 6 just because I have no experience and basically got the CPAP on my own after my sleep study just because I was in the mild category yet still feeling symptoms. I also kept it lower because I definitely experience aerophagia if it goes much higher and I thought maybe the air pressure was leading to my central apneas. The Obstructives and centrals are anywhere from 10-15 seconds however there are a couple centrals that are about 25-30 seconds - hence me trying to make the low pressure work!

So this is an example of me on auto from last night my AHI was only 2.5 and I had it set on a range from 5-20. The aerophagia caused me to wake a couple of times and as you can see here are my events off to the left. It looks like on the graph it says that the highest the pressure got was 13/14 ish. This is also with No EPR. Thanks again for your input.

RE: Help reading my Oscar Results
Your pressure range of 5-20 is too wide open.  You may do better with a narrower pressure range.  More like 6 min and 10 max.

I was unable to tell from your graphs, but the flow limitations are more than likely driving your pressure.  EPR controls flow limitations but can also increase clear airways.
You would have to experiment to find that happy balance.

Try 6-10 with EPR 1 and see how that feels.  Note if you still swallow air and at what pressure.

When you post a new chart, please format as you did in your first post, but eliminate the calendar and pie chart.
If you use the F12 key (on a Mac, Fn+F12) Standard View, this will format somewhat.

We need to see the following graphs:  events, flow rate, pressure, flow limitation and leak rate.  Eliminate the calendar so that all of the statistics in the left sidebar show.

How do you feel this morning?
Any aerophagia (gas from swallowing air)?

RE: Help reading my Oscar Results
Did not wake up from the air swallowing but still wake up pretty foggy and not very clear for the most part. Usually takes me a few hours to snap out of it. I did notice I had my longest obstructive and central time in the past couple weeks

RE: Help reading my Oscar Results
To verify, the FL are driving your pressure up to maximum, and you are probably experiencing arousals that you don't remember, but does affect how you feel the next day.

If your willing to experiment, let's narrow the pressure a little more.
Try a minimum of 7cm with a maximum of 9cm. Try to keep EPR at 1.
