How Does Auto Set Peak, Really Peak

How Does Auto Set Peak, Really Peak
If the CPAP is set at 10, it will stay at 10 all night.  If it is changed to be auto betwen   5 and 20, why wouldn't it go up to 20 at night when it senses apnea...my machine was just changed to the auto and my AHI was 8.66 and yet the pressure only peaked at 14.  I'm wondering if it should have gone higher right on up to 20 or if the auto set should be for 10 and 20 to help get my AHI down.

RE: How Does Auto Set Peak, Really Peak
The auto cpap relies on certain conditions being met for pressure to be increased and decreased. Those conditions are mainly based on specific feedback from the flow and pressure sensors. It's complicated to say the least, and much of it is proprietary and the subject of patents. If events detected by the sensors do not meet the criteria of the algorithm, pressure will not increase. If no events are detected for a period of time, pressure may be decreased. An obstructive apnea may increase pressure after the event by 1.5 cm, but the timing of subsequent events, flow limitations, snores etc that determine the rate at which pressure increases or whether it increases at all.

You don't mention they types of events that make up your AHI, but centrals do not increase pressure, and hypopnea results in lower increases than obstructive events. We also don't know what range you are using for auto CPAP. It is preferable to use a narrower range, or at least set the minimum pressure high enough to prevent most events. Rather than discuss the response of your machine in general and abstract terms, why not post a chart, and we can better explain what is going on.

RE: How Does Auto Set Peak, Really Peak
The Auto algorithm will only raise pressure as high as is needed.

Also, it depends on the breakdown of the AHI. If you are experiencing mostly Clear Airway events, the auto algorithm will not treat Clear Airways, therefore will not raise the pressure no matter how high you set it.

You may want to consider downloading #SleepyHead software, and start looking at your data to see what apnea events you are having. That can help determine what your pressure should be set at.

Read the tutorials in my signature line and post a SleepyHead screenshot here, then we would be able to better answer your question.

I'm still trying to figure out how to make the screenshot show up here and not just the link.

For last night, I was changed to an auto of 5 and 20, and I think it should be more like 10 and 20.

Your never went over 14. You could probably drop it to 15. Starting at 10 looks good.

RE: How Does Auto Set Peak, Really Peak
This is the graph we need, showing pressure response to events.  Please consider changing minimum pressure to 8.0.  EPR at 2 may be fine, but time will tell.  This is normal operation for your machine. Pressure peaked following multiple OA and H events, but did not respond to CA.  With a minimum pressure of 5, your machine simply has too far to go to get to where it needs to be when obstructive events begin.
