I took the suggestions and got a soft fabric covered cervical collar and modified it to two inches wide By doing this i have avoided the use of extended velcro tabs.  Any suggestions as to my next steps would be welcome again

You're still seeing those pesky clusters of events -- darn! Any chance your chin is sneaking under the collar? If not, the two other common culprits are REM sleep and back-sleeping. The timing of your clusters doesn't really look like REM to me. You may have previously posted about this, but in what position(s) do you sleep at night?

Two other questions: do the large leaks wake you up, do you think? And what are the breaks in therapy -- maybe bathroom breaks? If so, you might try doing more to restrict fluids in the 3-4 hours before bed, especially coffee and alcohol, which are diuretics.
