Full face masks push on your lower jaw. This might move the jaw back and worsen your apnea a bit and require some more pressure.

since its an older machine i would bet the bank its not putting out the proper pressures which more than likely explains all your symptoms as you are not sleeping well through the night

The goiter alone not "could interfere" it will certainly interfere! Speaking as one who has dealt with goiter and 2 surgeries to remove it (wouldnt let em go all the way the first time and ended up doing so 10 yrs later...) It got big enough before the second surgery to displace my trachea (they removed 94g) getting that out of there was worth it... You hear all those stories about people not doing well on replacement hormone... It works way better for me than the natural stuff that diseased organ was pumping out. Smile

From what you wrote, I would guess that you do indeed have higher pressure needs. (might even have contributed to the "stuffiness") The data capable auto machines will tell you what you need for certain, and they will tell you how well it is working. I can't imagine treating apnea without that.

Tongue Suck Technique for prevention of mouth breathing:
  • Place your tongue behind your front teeth on the roof of your mouth
  • let your tongue fill the space between the upper molars
  • gently suck to form a light vacuum

Practising during the day can help you to keep it at night

I find the AIRFIT F10 full face mask exerts very minimal pressure on your jaw. When too tight, it tends to press on the cheeks either side of the nose.

If it did press your jaw back, this might impact the airway in throat region, so good point to consider.

To be clear, I don't know how common it is or how bad it is when it happens. Some of the sleep lab folks over at binarysleep web site think it's a problem. (and some don't.)

I use the Mirage Quattro at 20 cm/H2O pressure which requires pretty tight lower straps. I do not think that the mask really moves my lower jaw back. I could be wrong but I do not detect it. I think that this is something that may well vary from person to person

On the other hand the larger air space inside a full face mask acts as a buffer to pressure and flow changes being recognized by the CPAP. I think that the changes make it back down the hose to the sensors in the CPAP but slower than with nasal pillows. I think that this is what may be part of the cause of the apparent lower pressure required by nasal pillows.

My thoughts for whatever they are worth.

I guess I’m also a Neanderthal or as my granddaughter likes to call me, a “Dinosaur”!!

I was diagnosed with OSA in 1998 and was given a CPAP system. At that time there was only 1 “Sleep” doctor in my area and the sleep lab was a hospital room, which with all its clinical coldness, I defy anyone to get even close to a night’s sleep in!!

My first machine was a Teledyne that was basically a square box with an air pump and filter. The doctor had the personality of a gnat with the attitude “I’m a doctor and you do as I say” basically “I’m God” and it took months to schedule an appointment with him.

The CPAP made a difference but after a while I felt it wasn’t totally effective. The machine had 2 rotary switches, hidden behind a plate that was easily removed with a small Philips screwdriver. One switch was for Ramp Time and the other Pressure. I started adjusting the settings and developed my own technique of evaluating the results. Basically, if I awoke with a specific soreness in my throat, I increased the pressure. (No such thing as Data Recording and analysis software back then)

After 2 years I finally went for a follow-up and the Dr. went ballistic on me for changing the settings from his Rx. He sent me for another sleep study and you guessed it, my settings were correct!!

After that I basically wanted nothing to do with him but in 2008 the motor of my trusty Teledyne burned out and I was forced to go back for a new Rx so I could get a replacement. My replacement was a ResMed S8 Elite II but I found instructions on accessing the “Clinical” menu and continued merrily on my way without ever having to see the SOB again.

Fast Forward to last month; the S8 ended up with the famous Error Code 1019 and no amount of drying with a hair dryer helped. (I did rescue it a few years ago with the dryer trick after my wife accidently moved the unit with a full humidifier tank)

I went to my family GP and he told me that the old SOB, I hated dealing with, had retired and today there are 3 sleep clinics and multiple Apnea specialists available. I went for a sleep study in a brand new ”State of the Art” facility and was introduced to a great new doctor who was impressed by my commitment to CPAP and ability to regularly monitor my condition. He recommended a new AirSense 10 which I can monitor myself using MyAir and Sleepy Head software.

Wow the CPAP world has definitely changed over the past 17 years and now I’m finally caught up to the new world.

I’ve used my new unit for 2 nights now. MyAir reported a score of 98 yesterday and 100 today. It’s too soon to start reading the SD card but I’ve joined this forum and have installed the ResScan & SleepyHead software.
