I got a request from my sleep specialist.
Hi Everyone,
Its been a little while since i have been on here. All is great in dreamland and me and the damdman are buddies again. 
I went to the Sleep Dr today. I seen his nurse or pa. She noticed I had the pressure cranked up to 20 and she asked me to please not adjust preassures. Even said i could blow a lung up. I just said ok and thanked them for all there help. 
I guess ill retweak it monday or tuesday. 
Any thoughts on best way to handle this? They are moving it to a max of 16 from the 20 i had it at. She said my average was 14.6.

If you need more than 16 cm pressure, you probably need bilevel anyway. Your pattern has been one that shows intermittent obstructive apnea and we were working on a soft cervical collar solution. Your AHI tends to be under 2.5, so the sleep doctor is probably happy with that, and the mechanism of your apnea (positional) is not likely to change by increasing pressure. Go with 16 cm max.

You couldn't 'blow a lung up.' Impossible, even at the maximum pressure a xPAP machine can produce. Your provider lied to you. And she knew it was a lie too. I'd dump these people. If they'd lie to you about this, what else will they lie to you about?

Me? Can you explain how a CPAP that generates 0.284467 PSI at the Max of 20 cmH2O could blow out a lung? I don't appreciate scare tactics or propagation or lies. You have lost one patient. Bye bye.

Wait... what?  You mean that medical professionals would actually use scare tactics?  Or propaganda?  Or outright lies?

I've certainly never in my life heard of such things!      I think we should get on our knees, and bow down to these medical deities like the uniformed, ignorant peons we are.  Certainly they have only our best interests at heart, dontcha think?  Bigwink

Yeah, Dave, but I think that the education a lot more of us got from Fauci is more "Who Ya Gonna Believe, Fauci Or Your Own Lying Eyes 101"

I was really proud of myself when the RT at the DME was aghast to find out I was adjusting my own pressures. She said, "are you a respiratory therapist or a doctor?!?!" I replied, "No, but I do understand tuning and forcing data. I lost 130 lbs that way. What's your superpower?"

Medical professionals always say, "you had bariatric surgery, right?" and I tell them, no, I just took control of my own health. And I'm not giving it back anytime soon.

Oh, and B68 -- 3 weeks ago I cured my apnea by following Sleeprider's suggestion about the cervical collar. Listen to that wise advice, and check your numbers to see if it works. I classify this as yet another smashingly successful data-driven seizure of control over my own health!

I was wondering why she would do that. I guess they are into the control thing.
Sleeprider I do need to get back on the collar search. I have been enjoying the 8-9-10 hr sleeps i have been getting. They are soooo nice.

You still here? I thought you blew out a lung.

Talk to Cathyf about a collar. She kind of gets it.  As long as you see obstructive clusters, you need to get that system optimized.

There's an online soft collar you can order called Corflex Ultra Cervical Collar. It appears to be comfortable and effective for combatting positional Apneas, according to at least one Apnea Board member.
