I made the changes to my charts, last night was just under 11AHI. My wife uses the same machine at about the same setting, the only diffrence is she uses a Wisp mask. Don't think I can use one. I tried it on years ago and got very claustrophobic. Her AHI numbers are consistently below 1.

Not sure what changes you made, but the Obstructive events on your graph are clearly positional.
Have you tried a soft cervical collar yet? I’m glad your wife’s therapy is going well, but don’t compare yours to hers. You are two different people. Your pressure continually maxes out at 15. I might almost agree with a prior suggestion to set your max pressure to 20, except that you may not need that high pressure if it’s just a matter if wearing a collar and taking care of the positional problem.

Had a good AHI last night, under 3. Tried to position my pillow a bit more under my shoulders to keep my head tilted back. Last three nights results attached.

That last chart looks good.
You still may need a bump in minimum pressure, but wait until you try out the collar or chin strap. Then you’ll have to determine what works best for you, and more importantly how rested you feel.

Using a pillow for positional apnea can work, but since we move in our sleep, it often moves out of position. In my case, I find when I lay on my side, pulling a corner of the pillow over my shoulder and under my jaw is ideal. On my back, I avoid having the pillow prop up my head, and instead try to have it support my neck comfortably. I think the soft cervical collar will wok for you, but don't use the chin strap until the collar fails. The problem with a chin strap is that they pull the jaw back and put pressure on the tissues under the jaw which can make positional obstruction worse. dry mouth is often if not almost always from mouth breathing. I can attest to the value of the c-collar in keeping mouth and lips closed, thereby reducing leaks and dry mouth. I still get some lip leaks with the c-collar but nothing so bad as without the collar, during which leaks are bad enough to make cpaping nearly useless. keep an open mind. it takes trial and error and perseverance to clear the early stage hurdles.
