Need advice with BiPAP, no OSCAR data yet

RE: Need advice with BiPAP, no OSCAR data yet
Weiss27md, I'd like you to go into your settings and scroll down to Essentials. Be sure the setting is set to Plus, not just on. With Essentials set to plus, your on-screen sleep information will provide average pressure, average expiratory pressure, inspiration time, expiration time, I:E ratio, Minute Vent, Tidal Volume, Spontaneous cycle %, AHI, AI and Central AI. These can be viewed for the last day, week or month. We can determine what kind of events you have using your on-screen data if you will copy this information and post it.

Sleeprider, thanks I will do this. I just went back to my sleep specialist today. They said my sinuses look good and don't see any issues 3 months from the surgery. I did get another at-home sleep study so I will be doing that tonight. I will be back to BiPAP tomorrow night.

One thing you mentioned was the dry throat. This can be a sign of mouth breathing which would definitely affect your therapy and results. There are some simple fixes for this if that's the case. When you get your PC and supply us with data it will help determine if mouth breathing is an issue.

Got my results from the at home sleep study that I did last night.  https://imgur.com/a/Spyw76E

AHI has increased to 8.3
Central 1.6/hr
Obstructive 2.4/hr
Mixed 1.6/hr
Hypopneas 2.6/hr

I will have in lab study done but that might be months away.

I think it stated there were 19 OA to 13 CA and whatever Hypopneas. The Impression area is absent of CA entirely. Another Doc DoLittle is blind to CA events.

I did use CPAP mode last night. Started out with 7 like suggested but felt like too much pressure. Went down to 6. I do remember waking up several times and feel worse than my first day on BiPAP. My sleep tracker also shows less than an hour of deep sleep. AHI was low but I feel like crap.

If I read this right, AHI was 12.8 and is not successfully treated as is. To earn the claim medically treated, you need to lower AHI to 5. Here on AB, users feel better with an even lower AHI of 2-3.

CAI of 9.6 is still pretty high.

Large Leak at 25 L/m may indicate there's a leak that needs addressing, either mouth leaks or improper mask fit or adjustment.

Combined or individually, that is why you have the result "feel like crap".

Something, or several somethings, need to improve for you to benefit from therapy.

Ask your doc to please review the stated diagnosis of OSA.
Say this, in telecommunication then it is in writing

'hey doc, would you please review my Alice test and the diagnosis of OSA.  I don't question that I have OSA but If I'm reading it correctly I also have Central Apnea and Mixed Apnea per (cite the findings on the Alice report) I do want the correct diagnosis so could you please confirm.

His response will do a lot to determine if he is a Doc or a Duck.

RE: Need advice with BiPAP, no OSCAR data yet
I found out that I do have mouth leaks, even on low pressure. Tape did not work well. I have stubble and it does not stick well. I would have to shave every night with how bad my stubble is. I have ordered a soft cervical collar (could not find the ones recommended so got a 3M one that looks good), a new mask (N20) and a hose clip. Windows laptop should be here Thursday so hopefully have OSCAR data then. Tried sleeping on my side the whole night last night, did not work well, kept waking up and had mouth leaks. Even with low pressure.
Mode VAuto
Max IPAP 14
Min EPAP 4
PS starting with 4 but eventually lowered down to 2

Any recommendations on pressure settings till then? Tried CPAP mode starting at 7 and lowered to 6, still bad sleep.
