I found someone on my local online marketplace selling an ResMed Aircurve 10 ASV for $2500 CAD. Wish I had the disposable income to just buy out of pocket and not have to jump through all the hoops.

Had an interesting phone call from my sleep clinic/DME today. He told me he noticed I've been changing settings and wasn't happy. I told him I'll always have a hand in optimizing my therapy and he needs to be okay with it. He kinda backed down after that. But then he proceeded tosay that me lowering my max pressure setting is likely the cause of all my centrals...

I wish I didnt already get my machine from this company so I could switch.

Then it appears the doc doesn't know as much about centrals as he charges you for.

Ask how decreasing pressure which decreases CO2 washout causes more Central events? I'll guess, his name is Doctor Knownothing.

Thats one of the issues, he's a sleep clinician and not a doctor, so I guess it's Mr. Knownothing.

One of the downsides to socialist Healthcare. You don't get much of a choice where you get referred to.

He essentially reverted me to the out of box settings on the machine to "let the machine do the work". I'm going along with it as I see this as my only route to getting him to agree that this isn't suitable treatment and I need something different.

Is there a strong correlation between mask leaks and centrals/clear airways? The clinician is convinced that they're only happening because I'm having some mask leak.

He changed my settings to be min 5, max 12, epr 2 (only went along with this since I'm trying to be compliant since he holds the cards at the moment). I felt starved for air all night and had a terrible sleep, slept about 5 hours in and out of sleep. However, I had a good AHI (1.84). So he called me the very next morning so smug and was like "It worked, looks like we fixed your centrals!". Except I don't think it's appropriate to make a broad statement like that based off a single nights data.

Anyways, last night was the first night with a new mask (F30), so I had a pretty consistent but minor leak while I figure out the new fit (median 2.4, max 7.2), and my AHI was 4.66 (which is right around my 30 day AHI average of 4.43). He called today and said that those centrals are most likely all just due to leaks and he's still fixed them.

Attached the 2 nights mentioned above, and excuse the time shifted CA flags, I somehow messed up the flags by adjusting the time on my APAP.

For one, Min pressure and EPR 2 will not work mathematically. EPR is attempting to lower pressure by 2, but 5-2=3 and your PAP can't drop to 3. To fix that and you're feeling of air starvation, increase pressure from 5 to 6.

CA, as far as I'm aware, cannot be CREATED by leaks. Leaks can hinder Apnea treatment. So I'll call this info from this supposed technician as bogus. And you're correct, ONE night didn't make a trend. Tell him that, and what your next night holds, which may be CA all over the place. Maybe this tech has some brain freeze.

And you already know, AHI alone didn't tell the whole story. How did you feel afterwards?

So the grade for this tech? 0. He's looking for one good night to base handling years of therapy. He's a moron, and wants you to be duped into believing he knows something. If you go down to his level, he'll beat you with his superior mastery of stupidity.

So bit of an update. Tried the F30 mask, leaked 0 some nights, then major mask leak other nights. Also, the lower mask straps have given me some killer neck pain...so back to the P30i!



Anyways, question for if it would be fine to turn EPR back on to 1 or 2? I've been having EPR off and there seems to be no reduction in my CA's. Attached is last night with EPR off.

Little update, been rolling with the settings min 6, max 12, epr 2 for 2 weeks now and averaging 2-4 AHI, 99% centrals still. Have a referral to see an ENT to see if there's something up with my upper airway. Positive news though, last night I actually slept through the night completely.

Question, it seems like my flow limits are spaced out fairly evenly in 2 hour sections. I'm assuming that I'm getting them only in a certain sleep stage? Attached my last nights data to show this.
