New Machine Started Fine, Now Not So Much
Hi everyone,

I got a new ResMed AirCurve 10 ASV machine last November. The last one worked great, with AHI routinely almost 0, but it started wailing like a banshee, so it needed to go.

My new machine worked fine at first, and supposedly had the same settings as my previous machine, but my AHI routinely started climbing higher over time. I have no idea why everything was just fine with the old machine, and then I suddenly started having issues with aerophagia and feeling like my head was going to pop.

The doc did reduce the pressure, which included changing the EPR from 1 to 3 (as I just found out from my equipment supplier), but past that told me that less than 5 AHI is "treated" (thanks so much for the help, doc). I still have issues with aerophagia even with that change, unfortunately. Lately the AHI has wandered around the 5-10 AHI range and trending upward.

Aside from the increasing AHI number, I also started having issues with the machine at first starting gently (more or less following my normal breathing pattern), but then after few breaths in, suddenly increasing the pressure much higher, all while setting up a breathing cadence faster than my natural breath. I feel like I’m fighting it and have to blow back to get it to eventually get it to calm back down and more or less adjust back to my breathing cadence. This happens every time when I’m awake, and I’ve tried it sitting up, laying down, etc—the behavior never changes. (I don't have the ramp setting on at all btw.) Does this part of the issue possibly have anything to do with the EPR change??

My next appt with a new sleep doc isn't until 4/25, and I think I'm going to go crazy if something doesn't give before then. Thanks so much for your help!



FYI I included an overview chart for the time I've had the new machine so far, a recent night of sleeping fun, and a zoomed in chart showing the sudden ramp-up in pressure (and eventual calm down) during the first several minutes of turning on the machine.

I’m not a AVS user so take this with what others say but it looks to me you are having positional apnea. Also known as chin tucking, where your chin drops to your Sternum cutting off your own airway. 

Have you changed sleeping position or pillows?  If you did not have it before then it should be fairly simple to fix.
Apnea (80-100%) 10 seconds, Hypopnea (50-80%) 10 seconds, Flow Limits (0-50%) not timed  Cervical Collar - Dealing w DME - Chart Organizing

Ah ok…I think you might be on to something. I just tried turning on the machine with no pillow and laying flat on the bed, and it did not do that crazy ramp up pressure thing, so I’m going to try taking stuffing out of my pillow and testing that tonight. Thanks!
