If the CA you have is treatment emergent, they do typically reduce within 3 months.

Ok so only time will fix that. Tonight I'll do what I did last night, except maybe I'll turn off C-Flex. Thanks, will update tomorrow.

Hi everyone,

I've attached my last three nights of sleep. Last night was on 10 pressure, night before was 9, and night that before was 8. All with C-Flex 2. All on my side (with a backpack on), soft cervical collar and a breathe right strip. I feel the same as before CPAP.
What would you recommend? I'm starting to feel a little lost.

All help is much appreciated.

It looks as though a pressure of 9 or 10 works better for you. You are still having a scattering of obstructive events, but I would suggest you stick with 10 for a few nights and see how things settle in.

More broadly, I can easily imagine -- been there myself -- how eager you are to see change and how frustrated you are that you still feel the same. But improvement in the way you feel may come gradually, so patience should be your watchword. And bear in mind that only recently have you been getting the really good numbers.

So I would recommend sticking with 10 for a few nights and seeing if there's any kind of trend. Depending on how it goes, you may want to try 11. But really, settle in with 10 before trying an additional change.

Thanks Dormeo for the support.
Also I wanted to mention, my AHI at the sleep study was 1.9, and since I'm 16, thats mild sleep apnea. Also, my AHI was 7.2 in REM, and I had reduced rem sleep, so I think the real issue is not being able to have enough rem. So for me I don't think I'm going to be able to succeed with the traditional low number of like 1 AHI, I would guess I need closer to 0. Im not really sure though. I presume 11 would be the next step, and I'm very eager to try it, but as you said it might be best to stick to 10 for at least another night.

Help With Sleep Study Results
Hello,

So I was wondering if I could get some insight on my sleep study. I'm not sure what to make of it. The first specialist said I don't have anything, the second said since I'm 16 the bar is 1.5 AHI for mild sleep apnea, so I have it. Go titrated. I have a CPAP and have been using it for just over a week, adjusting settings with the advice I've gotten from another thread. I've just been doing some research and reading on this forum, and since I haven't felt any better (I know this can take a while, I'll be patient) I wanted to maybe find out the more specific issue I'm having so it can be properly treated - if that makes sense. If there is any more info that would be useful let me know. I'm really trying to get to the bottom of this. I felt another thread was necessary because I think this may be a different topic. If not I apologize.

The sleep study says I have no CAs or OAs, but I have them with my CPAP? I know the machine can give false positives for events (Some of them are) so I reviewed them myself and some seem to be actual apneas. Also, I kind of suspect UARS, but the study says no RERAs, so I assume that theory is out the window. What about the EEG arousals, could that be something? I know I'm rambling, but I can't stop thinking about this. 

Additionally, my AHI is almost always higher w/ CPAP? My first night was 5. Now I'm hovering around 1-2ish, but that is pretty much the same as my diagnosis. 

Some more info:
- Overbite, underdeveloped jaw
- Short tongue tie, was cut in 2017, still short
- My tidal volume on oscar has these giant spikes into the 1000s, which concerns me. the other breathing charts look abnormal too
- I was prescribed 6 pressure CPAP, now I'm at 10,
- Started C-Flex at 1, now at 2
- I recently I've been sleeping w/ a backpack on to keep me on my side, a breathe right strip, and soft cervical collar.
- Symptoms are basically 24/7: Tiredness, headache, brain fog, memory issues, unrested in the morning etc.
- No meds, I get avg 9hrs sleep. Can fall asleep fine
- No issue wearing CPAP, hardly even notice the pressure
- Random, but my I have woke up with my teeth hurting a little the last two nights, but never before this

All help/feedback is much appreciated, thank you

Added last nights oscar chart. I've noticed I wake up a lot (or so it seems) based on my flow rate chart, but they don't seem to be reras, although Im not 100% sure what that would look like.
Note: Went up to 11 just out of a gut feeling, numbers wise it turned out well, although im not sure I feel much different.

Your chart shows 3 flagged RERAs, the yellow vert line in the events (RE) chart and the Flow Rate. RERAs are defined as a series of Flow Limitations ending in Arousal. You may have more but that would take a detailed analysis of the flow rate chart looking at 2-3 minute windows for the 'clipped breaths ending in recovery, a larger than normal breath.

Hi Fred, I took a look at what the machine flagged as RERAs. The first one was when I was awake, and the second two dont seem to be reras (attached). Most of the times there is a disruption in my flow rate graph it looks something like the other attachment. Sometimes it gets really 'messy' with a lot of spikes and valleys.

Technically, you don't have obstructive sleep apnea based on the test result, and your AHI is low with CPAP therapy at pressure 10 and CFlex 2. There are sporatic OA and H events that are not clustered, which leads to the question, why are you using a backpack and soft cervical collar? Based on numerous spikes in the respiratory flow rate and reflected in the tidal volume and minute vent charts, your sleep is disrupted by numerous arousals. To understand better what is going on ahead of those spikes, we will need to see some screenshots zooming into about 2 minute duration segments where we can see the shape of the flow wave. That will let us know if you are dealing with a flow limitation or RERA issue.

I would like to see you discontinue use of a backpack, Breathe-Rite and soft collar so we can compare results. Let's simplify your therapy as much as possible and only add back what is needed. I think those additions may be more disruptive to sleep if they are not needed. Also want to verify which Dreamwear mask you are using. You don't have leaks, and say you're using a Breathe-Rite strip, so I suspect it is the full face model. Did you ever try nasal therapy with nasal pillows?
