Without further information I don't think I would change any machine settings except for turning flex off (unless it's off already).

It's an example of a natural law of Usenet discussion groups, from long ago, whose name I can't remember: When there's a request for advice that isn't being answered, the quickest way to get it bumped up to the top of the general attention pile is to give some bad advice, which will then be corrected by other people.
So from here on, the standard routine is: make the recommended change (in this case, the collar), post the new Sleepyhead charts, ask for further advice from the chart-interpreting experts, and continue that cycle until you've got low AHIs, comfortable & non-leaky mask, and restful sleep (no longer waking up tired).
About the tobacco smoking, OK I won't tell you that you need to quit, even though it's not exactly compatible with CPAP, but a quote from a wise guy (Kinky Friedman or one of his cronies) comes to mind: "Sometimes you just have to find what you like and let it kill you."
