I haven't followed along but did a quick look at your sleepyhead chart. You have large clusters of Obstructive apnea which can indicate positional issues. Normally that's caused by your chin dropping to your chest and cutting off your windpipe. I think a soft cervical collar would help cut out a lot of the obstructive apneas. It could also lead to lower pressures needed which might help with the clear apneas.

I agree with Walla Walla and ditto his post recommendations.

Without further information I don't think I would change any machine settings except for turning flex off (unless it's off already).

To Starberrylane: OK, there you go, and that's a start. It's an example of a natural law of Usenet discussion groups, from long ago, whose name I can't remember: When there's a request for advice that isn't being answered, the quickest way to get it bumped up to the top of the general attention pile is to give some bad advice, which will then be corrected by other people.

So from here on, the standard routine is: make the recommended change (in this case, the collar), post the new Sleepyhead charts, ask for further advice from the chart-interpreting experts, and continue that cycle until you've got low AHIs, comfortable & non-leaky mask, and restful sleep (no longer waking up tired).

About the tobacco smoking, OK I won't tell you that you need to quit, even though it's not exactly compatible with CPAP, but a quote from a wise guy (Kinky Friedman or one of his cronies) comes to mind: "Sometimes you just have to find what you like and let it kill you." Sad

Did they tell you what the CPAP compliance period is for Medicaid? It's ten months of at least 5 hours/night and at least 70% of nights, as reported by the machine (the reporting happens daily, to the DME, via the cellular modem in the Dreamstation). Otherwise the bureaucrats get to take back your machine. That's something to keep in mind, and to work on if necessary. (For insured patients who are not on Medicaid, when there is a compliance period at all it's any 30-day stretch within the first 90 days, and a minimum of 4 hours/night, not 5.)

The good news is that once you're comfortable with the machine and the mask and the pressure, compliance is no problem because you obviously want to be getting the beneficial therapy. Those of us who are CPAP converts (have got religion) would never want to be asleep without it. (See the Success Stories thread.) The bad news is that it does take some patients a while to get there. Getting fed up with it and wanting to pull the mask off in the middle of the night is a normal reaction during the first few weeks, and most of us go through that.

Very true. Sometimes things work out the way they are supposed to and a positive example of a catch 22.
