I left a print out of the screen shot and the doc called back, while he can manage most sleep apnea (he is an ENT), I'm beyond his scope and is sending me on a better sleep specialist.  Now I wait but at least I've moved this process up a few weeks.

From what I've read here, it seems insurance requires a test run with BIPAP before ASV and I'm fine with that.  My question for today is, I weigh 175 pounds, 6' tall, I do not have COPD, I'm wondering what the chances are BIPAP would work just as well as ASV for my condition, I can't imagine I would need much of a 'push' to breath.

Good for you! That was the right thing to do, and the correct response by your doctor.

You will fail BiPAP and ASV will be granted provided cardiac health is in-line with requirements (>45% LVEF).

I finally was able to grab the attention of the new doc and saw him today, wow, what a difference, it was so nice to have an expert to talk to. 

As for the thought on my 'Cheyne-Stokes' breathing, he doesn't believe it is true CSR; I don't have heart failure, I haven't had a stroke, it doesn't happen every night.  He feels it is more related to over pressure and positional....funny, I had just put down some notes that it appeared I was seeing CSR when I rolled back onto my back and no longer sleeping on my side.  He showed me my previous sleep study that showed my CA's go way up when I slept on my back, that was a surprise to me.

The doc has ordered a BIPAP titration study and mentioned ASV if that doesn't work...we are on our way.  While I'm waiting for my BIPAP titration he wants me to lower my pressure to 4 min and 8 max, EPR at 1, and asked if I need ramp, "are you kidding me, only going up to 8, I don't think so".

I'm going to bed tonight feeling better already, simply knowing that I'm moving forward again after a long wait has eased my anxiety.

That's great news. Although the pattern we briefly saw in your charts resembled Cheyne-Stokes, you didn't have the classic risk factors. In spite of many different pressure and positional therapy schemes, we were never able to get a consistent direction on your results here on the forum. I'm glad you now have engaged professional help, and that is really what we all hoped for you. Best of luck in this renewed pursuit of good therapy.

Did the doctor have any comment on the fast respiration rate?

No, I actually forgot to ask about my resp rate but after last night, I will more than likely calling him on Monday; with the lower pressure  settings (4 with max of 8), my rate was higher than what the 'average' has been, I also noticed my resting heart rate to be higher (went from 45 to 50).

I know that rebreathing CO2 is a possibility if the pressure is too low and I suspect I saw it last night, if I have the same results tonight I may turn my pressure back up or try the nasal pillow (with chin strap).  The last time I tried the nasal pillows I had too much leaking but the pressure was much higher, I am slightly curious if I could tolerate the pillows with lower pressure.

This may be too late to help me tonight; I've just been informed that there was a cancellation and I can go tonight for my BIPAP Titration study, YES, I'll 'come on down'!! 

What I'm curious about is how exactly does a BIPAP machine help augment a breath in the absence of respiration, it seems to me that it will only ramp up the pressure.  I know the ASV machines will push the entire tidal volume but I'm not convinced BIPAP will really help, unless too much pressure has been used that causes CO2 blow out and I don't think I ft that model, I've had my CPAP dropped down to 4 min and 8 max and it didn't help reduce my Centrals, nor what looks like CSR.  I'm guessing that this is still just the next step before the ASV is ordered.

BIPAP doesn't help you with CA's. It's just part of the insurance scam that runs our health care these days. You'll be one step closer though.

I had my BiPAP Titration study done last night and unfortunately I do not know the results yet, however, I had the best night sleep I've had an several years and woke up a happy camper.  I will wait a few days to call the doc and start asking for hints on the results.

I'll look forward to seeing your results. Without a backup rate, BiPAP is still an obstructive apnea solution like CPAP, but more comfortable. With a backup rate and some programming, it becomes more of a ventilator and adaptive servo ventilator. Hard to say what they challenged you with, but good to hear you felt better. That counts.
