The above answers are correct. I am not at all concerned you will develop a CA problem with pressure support. The first indication as you approach the apneic threshold where CO2 drops below levels needed to stimulate respiration, is a varying respiratory flow rate. This is sometimes seen as mild periodic breathing where the respiratory rate or volume oscillates. There is absolutely nothing to suggest that in your results.

As far as flow limitation, the pressure support acts like a light push from behind as you climb a hill. It helps to overcome the work of breathing against upper airway restriction. Flow limitation is not always from upper airway flow restriction, and there are diverging opinions of whether it is a priority to treat. Your tidal volume and minute vent are excellent, so there may not be a priority to overcoming flow limitation, especially if you are finding therapy comfortable and effective.

Ok, I see so the difference in pressure causes the airway to open up and kinda causing "venting" from the pressure increase. What would be a good ps point to try and reach than? Why would people find it uncomfortable, I don't get that lol at a PS of 5.4 I kinda like the ease of breathing, not that I have any breathing issues it just kinda forces you to take a deep breath. 

You guys are amazing on here!

PS: could that first cluster or OA be caused still by positional apnea? Maybe I have the front neck support too low.

This has finally dropped flow limit to manageable levels where you should experience fewer arousals. How are you feeling?

I’m feeling great! It’s been life changing as far as getting to work on time and saving my job. I can’t say for sure but I had advanced liver labs Before and after I started therapy and the labs were much better my doctor was shocked that the labs Changed so much just by getting on bipap therapy.

I still get a little sleepy but hey we’re talking about years of not sleeping well. I got thyroid and testosterone labs post treatment too and they both looked great (I have hypothyroidism and hypogonadatropism). I have also seen differences in my sexual life as well, morning errections we’re not a thing until now.

You have to remember my AHI was 84 my RDI must have been over 100 and my lowest O2 sat was 65. I’m only 25 and 260lbs 5 foot 9 I’m not hugely overweight. I wonder if surgery would help because I am able to make a snore even while being awake standing up.

Again I can’t say enough how helpful you all have been and you probably helped me to save my job!

I think we can call that a success and results like this are why we keep working with new members. Especially at your young age, it is important to have this under control, and it will both prolong your life and make it a lot more enjoyable. I'm not a fan of surgery, and you would be much better served by lifestyle and weight control. I'm hardly qualified to preach either one, although at your age I was a different person than I am today. Trust me when I say it's going to be easier for you to manage weight at 25 than at 65. Hopefully success with BiPAP will enable you to have the energy to do what's needed.

Agreed, I’m really hoping I can start loosing weight ASAP. For someone my age I am not the healthiest but a lot of my issues are metabolic or hormonal so I am really hoping that good restful sleep will help those ailments out. 

I really appreciate all you guys have done for me and the other new members. Hopefully I can study up more on how to interpret the Oscar results and be of help to the next new person. 

I posted last nights results, do you think I’m going to high on the PS because I did have two central’s or are they just there by change or misreported. I believe I had PS set to 6.6 but the graph will say. I feel comfortable with a starting min of 15 so it prevents early night events.

I'm not concerned with a couple isolated CA events. They don't mean anything unless they become a predominate feature of your therapy, and usually appearing in clusters as your OA was before positional therapy. I look forward to your continued participation here to help others. Young people often deny the possibility they can have sleep apnea, and you are living proof that it can be severe, and can be effectively treated. Your experience can help others that will relate more to your age, issues and results.

Your maximum pressure shows 25/18.4 which would be PS 6.6. The benefits of this higher PS are not apparent based on the 95% flow limit or results, so the decision to keep it is entirely up to you and how you feel with it vs PS 6.0. Your chart at PS 6.0 and this one at 6.6 both show a 95% flow limit of 0.12, and the difference in AHI is all on the cluster just before 01:00.

In other words, bumping up PS to let’s say 7 just to test would probably have no difference and it being at 6. I’m comfortable with the high ps I just don’t want to raise it to the point I’m getting CA.

I think it’s an understatement to call my case severe, it damn near ruined my life. I’m just hope that if my liver damage is correlated with apnea it will improve. I almost lost my job and I crashed a car and could have killed someone.
