That study doesn't differentiate between obstructive and central apnea so I am going to assume it could be either.

The AHI numbers don't make sense either.

56 AHI in supine position for a duration of 1 hr 31 min implies 84 apnea/hypopnea during that time.
5.17 AHI in non supine position for remaining 3 hr 16 min implies 16 apnea/hypopnea during that time.

84+16 = 100 total apnea/hypopnea divided by 4.75 hrs sleep time (4 hrs 45 mins total sleep time) should be an overall AHI of 21, not 6.7...

Not to mention the summary diagnostics portion in the middle appears to be a combination of sleep study and titration info which were studies performed on different days (6.7 AHI is from sleep study, 479.92 min sleep time matches the titration study). Even if I use your titration study length the AHI numbers don't make sense...

Assuming you have insurance that will cover a study you should bring up these issues up with your doctor and ask for an in clinic sleep study with titration including ASV as indicated for central apnea. State that your treatment has not been satisfactory and that your AHI remains high due to central apnea. Take some of your data from high central nights or run your machine at high pressure/EPR for a few nights to get some high central apnea AHI data if you need to try and convince him further.

Unfortunately it appears like too many others you have received inadequate investigation of and treatment for your sleep disordered breathing.

OK yep to the above. I'd add in make some notes on this uncomfortable, not treated status. Noting all aspects of the failure and telling Dr. Dolittle.

That is exactly what I was thinking.  If about 1/3 of the time is spent supine position then the minimum overall AHI would be 1/3 of 56.  Looks like I need to talk to one of the respiratory therapists to clarify and if not request referral to sleep specialist.

Just make certain you request specifically what Geer1 mentioned. You want a lab titration that progresses into ASV pressure, this is how I'd phrase it.
