I agree with Bonjour that your real therapy starts at about 1:15. My suggestion is:

Autoset mode
Minimum pressure 9.0
Maximum pressure 15.0
EPR full-time at 2.

Sleeprider,

I'm willing to try that and if it feels overly uncomfy, I can simply wind it back a bit. But yeh, I will do this tonight and post the stats in the morning and let's see how we do with that.

Thanks for the suggestion.

RE: Newbee - 1st Evening Failure
Even if you start with a lower minimum pressure, the Resmed Autoset will quickly put you where you need to be. Good luck.

RE: Newbee - 1st Evening Failure
Well ...  It's gone from bad to worse and i'm getting very concerned now. Sad Last night was a disaster.

First up, Min starting Pressure increased to 9cm. Max pressure increased to 15cm and EPR was activated, full time at 2. I must admit that at first I did not like it. I felt the EPR (I assume) cut in and when it did, the mask would sort of make a thud and it felt different to breathe. The air seemed warmer and stale and I felt ill at ease but after 30 minutes I certainly felt ok and I think that I actually liked it more than the evening before. I did feel for the first 15 minutes that it made my chest ache and gave me a chest pain, sort of burning lungs, but that passed. That also concerned me at first.

I got myself relaxed, prepared and settled down to go to sleep. I tried for around 45 mins or so and just could not sleep so I got up, laid and watched a TV Show on the floor, using pillows and a Doona, for about 30 mins or a bit longer and then I decided to try again because I was yawning and feeling tired. So mask back on and ready for sleep etc but it took ages.

Next thing I know, once again I'm jumping up terrified, scared as hell and panicking to breather, or fighting the mask. I don't know which. That did my mind in then. It was well after 2am, I was annoyed, tired, too scared to put the mask back. I had probably been to sleep for an hour or so.  I the decided to just lay down on the floor with some pillows in front of the TV and bingo, within 10 minutes I was asleep.

Now I have posted Oscar Charts and I can see that after the first session, when I was starting the 2nd session I had a a massive and continuos air leak and I am astounded that I did not notice this. I remember that the mask and breathing felt different the second session, but how I could not notice there was an issue and a massive air leak, I don't know. It is both annoying and disappointing that I was unaware of this.

I have posted Oscar Chart though there really is little data of any help. I think today I will try and get into see my Doctor and get a script for Melatonin as I have heard that will aid falling asleep. Having heart (AFIB) issues, they are very reluctant to issue anything that helps you sleep because obviously it can worsen events. Now I am starting to worry that every time I use the CPAP, I will end up waking up terrified and fighting........ At the moment I am not scared to wear the mask or use the machine, but that could become an issue if I keep waking up in panic mode.

I see that at 0144hrs I had an RERA Event. Is this what caused the panic??

RE: Newbee - 1st Evening Failure
Snooks, I'm sorry to hear about the rough night. I do think you'd benefit from rebooting. For maybe five days, stop trying to use the machine while you sleep. Instead, use it outside your bedroom at a time that's quite distinct from bedtime. Do something diverting while you have it going. Try to get in a couple of hours this way every day.

During this period, work hard on following sleep guidelines. See below. I have to admit I thought they were mickey-mouse, but when I got really desperate, I started using them, and I found that they made a real difference.

When it's time to try nighttime use again, set the machine up for normal sleep in bed (not the recliner). If you don't fall asleep within about 30 minutes, turn it off and sleep. The next day, do some more daytime use, and try again that next night.

It truly will get better. You are aiming to get your body to adapt to the new experience while avoiding setting up mental associations between using the machine and bad stuff.

Sleep guidelines:

• Keep a consistent sleep schedule. Get up at the same time every day, even on weekends or during vacations.
• Set a bedtime that is early enough for you to get at least 7 hours of sleep.
• Don’t go to bed unless you are sleepy.
• If you don’t fall asleep after 20 minutes, get out of bed.
• Establish a relaxing bedtime routine.
• Use your bed only for sleep and sex.
• Make your bedroom quiet and relaxing. Keep the room at a comfortable, cool temperature.
• Limit exposure to bright light in the evenings.
• Turn off electronic devices at least 30 minutes before bedtime.
• Don’t eat a large meal before bedtime. If you are hungry at night, eat a light, healthy snack.
• Exercise regularly and maintain a healthy diet.
• Avoid consuming caffeine in the late afternoon or evening.
• Avoid consuming alcohol before bedtime.
• Reduce your fluid intake before bedtime.

RE: Newbee - 1st Evening Failure
I understand the comments Dormeo and thank you.

I have made many changes, reduced fluid intake, now drink Decaf after lunch, etc. I see what you are saying about taking a break and rebooting and it is an option, but I really do want to try and beat this thing. I have a Cardiac Specialist appointment in 2 weeks and I want to take a report to him showing that I have achieved a goal. I know it sounds silly, but I need him to see that i am fighting hard and he needs to do the same (For me).

I just need to work out why I woke up fighting when I was relatively comfy in the mask all evening. I also think Melatonin is worth discussing with the Dr.

Thanks again and I certainly will keep this option in mind.

RE: Newbee - 1st Evening Failure
Well, i just came from the Dr's and he prescribed Melatonin after I brought it up. So it is being made up by a Compounding Chemist as we speak, 3mg and take it 1hr before going to sleep and try and take it at the same time every evening.

We discussed the side effects, headache, foggy etc and I get those now, so it really isn't any difference. A bit more attention to sleep hygiene and let's see how we go tonight Smile

RE: Newbee - 1st Evening Failure
Is it possible that my mask is not working correctly?  Yesterday we had that massive leak that somehow I failed to even realise and today, it did not feel right at the later stage.

I started therapy, took a while to get to sleep but I did. Things were going well until a big thunderstorm woke me up. This concerned me so i got up due to the thunder and lightning etc and unintentionally fell asleep watching the Weather Reports on the TV. I woke up an hour or so later and went back to the CPAP even though the storm was still happening.

For some reason, again, the air in the mask felt hot and stale and harder to breathe. I swear that the mask was not venting properly. I persisted a very short time, stopped the machine, took the mask off the headgear, checked it over put it back on and it seemed to be functioning way better then, easier to breathe and the air was cooler. But by then I was uneasy and took the mask off which was silly, but I did it.

Days ago I was reading in CPAP Mask Product Reviews a review on the Resmed Airsense F20 Airfit and venting issues and air direction were discussed, together with the possibility of the vent holes being obstructed. I wonder if that is what is happening here?

I recall the machine displaying events were very low when i finished the first session and at the end of the short second session, events were up to 17.62 which surprised me. Things don't seem right and I wonder if we need to change something.

Again, I did not do a long time of therapy but I am getting used to it and hopefully things improve soon. At least I am feeling much more comfortable and relaxed on the machine, especially at the start of the night.

RE: Newbee - 1st Evening Failure
This chart has a notable increase in CA events. It may be a one-off/one night thing. I would suggest keeping an eye out for the CA. I think it's likely going to be treatment emergent or maybe lots of breath holds while moving around. Even so, keep tabs on the event count. One consideration is to lower EPR for the time being to lessen the likeliness of these CA recurring if they're treatment emergent. And if these are treatment emergent CA, this will diminish within 3 months of PAP use.

Thanks SD94. That sounds very worrying and I don't like the sound of CA's.

I will turn off the EPR, I just wonder if the minimum pressure of 9 would be too high first up? I guess i can reduce to say 8 and then see how we go for a few days.
