Sometimes, unplugging it from one USB port and plugging it into another port will fix it. At least until you run out of USB ports. In my case, dinking with smart card services (or whatever it's called) fixed it.

My Computer/Computer is sometimes in the start menu instead of on the desktop.

There is a clear pattern. plugging in the reader with the laptop running will not allow the installation of the driver. If I plug in the reader, close the laptop, let it go into sleep mode, wait for some time, re-open the laptop, it loads the driver.
