Question about APAP machines and sensitivity to hypopneas
Hi, I've been on CPAP for 3 months now (ResMed AirSense 10 CPAP) and have had significant problems with aerophagia, even at low pressure settings (I've still been playing around with different pressures because the optimal pressure isn't clear in my case). I've read that Auto-PAP machines can help with this problem better than BiPAPs can, but also read that APAPs aren't as sensitive to hypopneas as apneas. I have significantly more hypopneas than apneas, so I'm just looking for advice as to whether certain APAP machines are better/more sensitive than others for detecting hypopneas? Thanks!

I have not read anything that suggests that the type of therapy (bi-level versus APAP versus CPAP versus fixed pressure) matters with regard to aerophagia.

I have heard that the pressure itself is what matters. Some are sensitive to higher pressures that end up being swallowed. Lowering the pressure temporarily to reduce or eliminate painful aerophagia is usually suggested. If pressure is increased slowly, most people gradually adapt and do not continue to swallow air.

Can you please summarize what pressures you have tried, and the results in terms of your comfort and AHI? Your profile shows 12, and ou don't specify which Airsense 10 you are using (CPAP, Elite, Autoset). Also, what EPR (exhale pressure relief) settings have you tried? Did that make any difference?

I went back to your post history, and I see you have the CPAP brick and are considering other machines. The very best machines for treating hypopnea with the lowest impact to aerophagia is a BiPAP. I believe Robysue also uses this approach and shares your problems. Normally these can be a bit expensive, but there has been a Dreamstation BiPAP Auto DSX700t11 listed on Amazon from $720 to $799 (price varies a lot). It's at $760 tonight. That would be an excellent choice that would allow you to set a low exhale pressure (EPAP), then add just enough pressure support to treat hypopnea, and it is a full data machine. I think you might need to use the EncoreBasic software as Sleepyhead has not caught up with that one yet. Supplier #2 has a DS760 BiPAP Auto for $619. This is an older generation that works just as well, but you need to add a humidifier, so there would not be any cost savings.

You could try any of the Auto CPAPs from Philips (Dreamstation Auto) or Resmed (Autoset), and those are going to range between $380 and $460. Impossible to say which one would work better for you, but the Resmed Airsense 10 Autoset might have an edge for hypopnea at low pressures.
