I do have a request submitted for a clinical manual for the VAuto, Walla Walla. Won't be touching those controls you mentioned until I've read up on them.

For auto-cpap, from machine data or software. You can set the min pressure 1 or 2cm below 95%. Or clinicians commonly use the maximum or 95% pressure for fixed pressure CPAP, this can also be used for min pressure. https://aasm.org/resources/practiceparam...rating.pdf

OK, folks, last night was the first night on this new machine. The numbers are terrible. First half of the night, up until around 3:00am, I used the Auto mode option. The settings used were: IPAP = 14 EPAP = 10 PS = 4 TiMax = 2.0s TiMin = .6s Trigger = Med The last 3 settings were originally set by the DME and not me. I left them untouched. First hour or so the pressure felt fine; the remainder of the time I experienced difficulty ‘getting enough breath’ at this setting. The AHI number turned out to be in excess of 21. At around 3:00am, I modified the Auto Mode IPAP and EPAP numbers to 17.0 and 13.0 respectively. All other settings kept the same. This change eliminated the feeling of not getting enough air. This setting brought my overall nightly AHI down to 15.76. The number of CA’s and OA’s were still considerably higher than the counts experienced using CPAP with pressures of 17-20. Not sure where to go from here. Is there some ‘auto’ tweaks someone might recommend? Or, should I step back and try the 17/13 PS 4 settings in ‘S’ mode that the DME initially set up? I haven’t tried sleeping with the ‘S’ mode setting because of how abrupt the inhale/exhale pressures felt in a few minutes of testing. The ‘S’ mode settings set by the DME are: IPAP = 17.0 EPAP = 13.0 Easybreath = OFF TiMax = 2.0s TiMax - .6s RiseTime = Min Trigger = Med Thoughts anyone?

Actually you were in S mode if you look at the data to the far left. I try minimum pressure 13cm, maximum pressure 20cm, a pressure support of 3, and finally I'd make sure it's in auto mode. The drop in pressure support is to see if it clears up the CA events.

I didn't know auto didn't have rise time, I would assume it could be fixed at medium and about 300ms. It could feel similar to an autoset and epr3 or an ASV
In S mode, turn the rise time to medium, to give a slower change from epap to ipap I agree in auto, to have the min at 13 and max at 20 and get some more sleepyhead charts up. I say this because you said you felt like you weren't getting enough air. ps3 probably will reduce the new user pressure induced CA, It can be the body adjusting the co2/o2 balance, waiting for the co2 to build back up to trigger you to breathe. One example that has used an o2 meter, showed that for them, there doesn't seem to be an o2 desaturation that go with these. I would put breathing easy above this. I would use 4 if it was better. Pressure induced CA can resolve over 12 weeks in most people. If you continue at 3.5 an hour for more than a few weeks and not reducing. It will need watching and referring back to the doctor. They may be a real central apnea with a o2 desaturation. The study should have shown if you had real CA

I agree with the other suggestions. Your recommended EPAP pressure was 13 cm and you setup your night at 10. So the OA events were more prolific than I would have expected, but they are not unexpected. I think you will do fine with Vauto mode EPAP min 13-20, PS 3.0. Your inspiratory time was relatively short and expiratory time long, so a closer look at the Flow Rate and events to see the dynamics of the apnea is something we might want to do.

You might like to increase your Ti Inhale max a little bit to avoid the abrupt change to exhale. I had this issue and changing the default (which is 2.0 secs) by half a second solved this problem for me.
