Find a different type, size or fit for the collar. The problem can be fixed, but it may take some experimentation. The problem here remains positional.

Definitely positional. There is no amount of pressure that will fix that.
The collar he tried may not fit him properly. Measure the circumference of the neck and add a couple inches. Also measure the bottom of the chin to the neck. A person that has a shorter neck will require a shorter collar, maybe 2-3 inches. A personal with a longer neck can get away with 3-4 inches. Measuring is important. Also, be sure the bed pillow is not too tall. It must be able to support your head and neck without causing your head to tilt forward into your chest. He has to work on the positional sleep issue first. It's possible that he wouldn't even need to use such high pressures.
