FAA used to rely on pilots self reporting OSA which caused too many to ignore symptoms. Then you were grounded for 3-4 months while your paperwork went through the system. That's tough on pros that don't get paid if they don't fly, so it was really tempting to avoid finding out.

Now the assumption is that if you meet certain BMI and neck measurement minimums then you need an OSA screening. If the screening is negative for OSA you are good. If not, you need a sleep study and 30-90 days of data. All this is at pilot expense. But now you aren't grounded while all that works its way through the system. The result is that pilots get treated and don't lose time. Sure, there are exceptions, but the attitude on both sides is much better.
