I have discovered what was obvious once I figured it out...

This will make immediate sense to anyone who has worn a bandana or other headscarf. The way you put the mask on is to pull the whole thing down around your neck, pulling it so it's underneath your hair (sorry, bald guys, you are out of luck!) Then take the top of the headgear and pull it up past your face above your head. The back of the headgear ends up underneath your hair, which helps to keep it down.

This works much better with the cervical collar -- the back of the headgear ends up underneath the back of the collar.
