Be careful of death by analysis. You have great results with consistently less than 2 AHI and a low level of flow limitation, all with very low pressure starting at 7/4 (EPR 3). You could stand to increase minimum pressure a bit. Once you're done chasing the other issues down a rabbit hole, start looking outside the CPAP therapy for solutions to fatigue.

Take whatever your doc says you should weigh and add 10-15%
There's a difference between 'lean' and 'skinny'...
