Thanks everyone. I ordered a soft cervical collar and adjusted the machine to the settings you've suggested.
By the way, I did not mishear Dr. Gold regarding his thoughts on a full-face mask. We had a quite heated exchange. In fact, a short Google search found another of his patients stating the same thing.

This is the existing data with the Snore and Flow Limit graphs added. I will post new data for the next 5 days. My soft cervical collar should arrive by Monday. I will indicate on the images which days I am using the collar. I also now have a pulse oximeter that is compatible with sleepyhead, so I will add that data as well.

You still need to raise your epap from the initial value of 5 to something nearer your 90% treatment pressure I suggest a value of 12

I'm going to run the following settings for the next 7 days, barring unexpected events.
IPAP max = 20
EPAP min = 12
PS = 4

My suggestion last page was PS 3 over EPAP min 10, IPAP max 20
http://www.apneaboard.com/forums/Thread-...#pid229615 . I think 12 is too large of a move. The Resmed Autoset will increase pressure if necessary, and has frequently brought your EPAP above 10. I also want to leave some room for the cervical collar to have an effect on the OA clusters. 12 may well be a good EPAP pressure, but the auto algorithm should take it there. This sets up a starting pressure of PS 4 over 16/12 and ignores the CA we saw in the charts.

With those settings, a soft cervical collar and my dental plate in, I am getting an AHI consistently less than 2. Without the dental plate and collar, I get massive mouth leaks and about 6 to 12 AHI.
I am not sure if I am prepared to wear all of this crap when I'm sleep.
