Can you post your sleep study results as well? The reason I'm asking is because most doctors only care about your AHI but the important number in my opinion is your RDI and I want to see if there is a big difference between the two numbers.

By the way, how tall are you? I want to see if your mediam tidal volume is still low. It gives us a good indication if you are still dealing with flow limitations or not.

besides the AHI report, this is the other page.  not sure if it helps.  my height is 165cm.

  1. your leak rate is pretty high
  2. your minimum pressure is pretty low
Normal recommendation on 2 is to raise the min by at least the same value as your EPR. So if your EPR is set at 2, you'd want at least 6. But looks like it should be higher still.

Guessing if you address those two things, it will help, but others with more experience can confirm and maybe offer more specific recommendations.

thanks this is helpful.   i guess i was wrong as i thought my leak was good.  how do you read that properly?  what is an EPR?

i only changed my setting once, where i change my max pressure from 16 to 19, keeping it at 4 though.  that was the original setting from the sleep therapist.

I don’t see a leak problem on the graphs you posted. Leak numbers are very low.

But your minimum pressure of 4cm needs to be higher to tackle the Obstructives.
My recommendation is a Minimum pressure of 8cm with an EPR of 2.
It should actually be set closer to your median pressure, but start with 8cm.

I misread your leak graph (read too quickly).

EPR = expiratory pressure relief; the gap between inhale and exhale.

If you have minimum pressure set at 4, you basically have no gap, and so get no benefit from it.

If you can tolerate not using ramp, then just turn it off. Many users prefer no ramp in their settings. It's a personal comfort matter, so based on your tolerance for CPAP pressure, the ideal is not to use ramp, but if you need it, then set to auto and as high of a starting pressure as you find comfortable.

i realised that i tend to have the highest OSA during the 6am and 7am , which is the time before just waking up.   not sure if there's a reason to this?  also should i get soft cervical collar or neck collar?

The easiest way to for us to interpret whether events are "sleep wake junk" is for you to post an OSCAR chart. Your description of apnea event just prior to wakening is typical of SWJ, but they are normally recorded as CA rather than OA.

This is somewhat normal for a lot of us.  I will see the most of my events during the last hour of sleep.
I tend to ignore them as they are usually triggered by REM Sleep or as we like to call it Sleep-Wake Junk.

A soft collar is what you should look for.
