If you have already been diagnosed with sleep apnea, there is no need to go through another home test. Your diagnosis doesn't usually change. If you had a sleep study in the past, which you clearly did, then you can just ask your doc for an apap.

I tried that. He didn't understand and took the week off. If you never hear from me again after Saturday it means I died during the unsupervised test!

Then you won't be needing a home sleep test.  Your doctor made an error, then skipped. Find a doctor that cares enough to do this right.  You have a diagnosis for obstructive sleep apnea.  The next step is either a titration test, which is a second sleep test performed in a sleep clinic with PSG that trials progressively increasing pressure to determine your pressure requirement; or the prescription for Auto CPAP and self titration.  Another diagnostic home sleep study is worthless, and the cost may be denied by your insurance since it is redundant.

If you have a copy of your previous sleep test, any physician (think of your primary care doctor) can write the prescription for an Auto CPAP, heated humidifier mask and supplies.  You should call your insurance company or check online for a list of in-network medical equipment providers.  Once you choose which provider that is in-network for your insurance you want to use, you can have them request the prescription from your doctor.  Talk with the DME and be sure you get the equipment you want (i.e. Resmed Airsense 10 Autoset with heated humidifier, heated hose, supplies and your choice of mask).
