Hi Lanco. From what I understand the Trilogy is a ventilator rather than a xPAP machine, and as such we're not really in a position to give any advice. You and your wife will need to work with her medical team to ensure that it is all set up and working as it should. Be assertive and don't let them fob you off.

Somebody might be able to advise about the costs and DME side of things - I'm afraid i don't understand the American system.

That is a true hospital grade ventilator and is most similar to the BiPAP AVAPS machine, but comes with a 5-figure price tag. We will follow along, but advising on the use of this machine is out of my league, and I would guess most others here. It can be monitored and set remotely, and her doctors have very specific objectives. If alarms are a bother, or sleep disruption, I believe they can be cancelled, but contact your doctor to have that done. Best of luck!

One think you can consider is that as a patient, it is her right to know and understand the therapy. She could ask for what her prescription is for the machine, including pressure settings, and she can ask how that setup is helping her to meet the doctor's objectives. If the cost of that machine to you is very high, then there may be alternative devices that may do the same functions for less. These are all things to take up with the doctor. As always, her medical records, such as prescriptions, diagnostics, and recommendations are all available to her if she wished to have copies and to understand this better.
